The only two tick dz that dont have pcr are
Anaplasma and lyme
Anaplasma phag is what rocky The fag had. What was wrong with him and what cells were they inside? What else was he probably co-infected with?
Fever and polyarthritis. They were in his granulocytes. A phag likes to co-infect with borrelia
Which anaplasma Attacks the platelets, therefore causing tcp and uveitis?
Anaplasma platys
Ehrlichia canis attacks the __________ and can cause any CS whatsoever
Monocytes
Babesia canis vs babesia gibsonni, which attacks grayhounds vs pittbulls?
Gibsonni is pits, it sounds tougher than b canis which gets gryhounds
How might a dog that hasnt been well taken care of present if hes had an overwhelming babesia infection?
In hypotensive shock
How could the hemolytic anemia caused by babesia result in hypotensive shock?
Hemolysis causes hemoglobin release Hemoglobinuria damages the kidneys. Damaged kidneys pee out too much water and the animal can become hypovolemic.
Babesia can be mistaken for imha. So what do you treat with if a dog comes in with severe hemolytic anemia in the south but youre not positive yet that its babesia?
Doxy and pred.
Babesia can rwsult in DIC. True or false
True
So in the beginning were treating a babesia suspect with doxy to cover our butts. We finally get our pcr back and its b gibsonni. Which two drugs will we put him on: A, A, I are your options.
Gibsonni get the two A drugs-- atovaquone and azithromycin. B canis dogs get imidicarb
Can you ever completely clear babesia?
No theyre always going to remain subclinically infected and Can suffer relapses so dont takr their spleen out...
(rem: relapse means theyre getting anpther infection from the same organism, vs reinfection, which is a different organism) |
Which three tick dz are setting up shop in their rbcs?
BBC. Babesia, bartonella, and cytauxzoon
E canis causes pancytopenia. Why?
Cause it attacks the bone marrow i guess
Acute cases of ehrlichia the dog will be "sick" but Survives, hence the overwheming number of pothounds that have ehrlichia but are still running around stealing food out of garbage cans. So theyre cute, we take one home with us. Whats she gonna develop years from now...poor Flo!
Chronic ehrlichiosis causes glomerulonephritis.
Are antibpdy tests specific for ehrlichia canis?
No those SNAP tests ae not diagnostic for e canis because they cross react with a shit ton of other ehrlichias.
If antibody tests arent specific for e canis then how do we diagnose them?
PCR .... Or presumptive diagnosis based on CS and response to Doxy
We suspect lyme dz because our patient has an acute shifting leg lameness and his joints are painful...and he has a fever of 105. Great. We do a jpint tap, what test are we gonna get informative results from, culture or cytology?
Cytology. Were gonna see a bunch of non degenerate neutrophils in that lyme joint. Remember borrelia is really hard to culture so youre gonna get a false neg joint tap n
What is the diff between lyme nephritis and GN caused by chronic lyme disease?
With a chronic lyme infection, the dog is making a lot of antibodies which are complexing with all the borrelia antigens those complexes float around and got lodged in the kidney causing GN. (theyre glomming up the kidneys). With lyme nephritis, the damn bug is directly attacking the kidney!
I used to vaccinate Abby against lyme. What two tests wont i run in the future, expecting the possibility of false positives?
IFA and ELISA. Shes gonna be seropositive for years from those vaccines
What test differentiates between lyme vx and infection?
Western blot, but its not widely avail. Pretty much diagnose them on CS and response to doxy
What is the lyme SNAP test detecting?
C6 peptide of borrelia
Um so what are the three drugs she lists as treatment for lyme?
Doxy, clavamox, azithromycin
What are the two tick dz that present in the summer, ie april to september?
Cytauxzoonosis and RMSF
Of the two dz that present in the summer, cytauxzoon and RMSF, which do you treat with heparin to keep the vessels open, and which would you not go near with heparin because the damn dogs already bleeding out?
Cytauxzoon treat with heparin because theyre prone to thrombosis. RMSF is causing spotty hemorrhages (petechiae) hence the name rmSf.
The cat has cytauxzoonosis so its gonna die. But what can we treat her with in hopes of being a hero and saving her life?
The same shit ypu treat babesia with. Either imidocarb or azithro and atovaquone.
RMSF is an annoying rickettsia that replicates in their endothelial cells. What does this explain?
Why their blood vessels get leaky and there is extravasation of blood.
(vessels are made of endothelial cells) |
We already learned that babesia can progress to DIC. What other tick dz can lead to DIC?
Rocky mountain
An initial leukopenia which turns into leukocytosis....thrombocytopenia...and anemia....what tick dz?
Rmsf. Platelets and rbcs are prob being lost trying to clot up all those spotty micro bleeds thats happening
In general (i think) IgG is indicative of previous exposure and _____ indicates a more recent exposure
IgM
What can we treat RMSF with since its a tick dz? What might we consider using in pups under six months old?
Doxy. Pups use enrp, tetra, chloramphenicol
Bartonella is famous for causing cat scratch but more importantly what does B vinsonii berkhoffii cause in dogs?
Heart shit!
If we see a boxer with a bunch of granulomatous shit going on, hes got a new murmur, and the question says were going to treat with azithromycin, what tick dz are we thinking?
Bartonella vinsonii berkhoffii
What does bartonella cause in boxers?
Endocarditis (whatever that is)
A dog can have bartonellosis for years and not present until ....
Stressed (parturition, co-infection)
Remember BBC tick organisms are hard to culture so will we be looking for bartonella in a blood or tissue culture?
No
Initially we treat bartonella with __________. What do we switch to if the dogs unresponsive to it?
Azithromycin initially. Switch to rifampin if the bartonella is unresponsive to it.
You kinda cant blsme the tick for causing _______________ because this time its the dog rating the tick that causes the disease!
Hepatozoon canis or americanuum
So hepatozoon is going to cause signs such as fever and stiffness in a waxing and waning manner. Why is that?
Cause the merozoites are fucking themselves inside the dog causing relapsing infections every few months weeks idk
What kind of radiographic change would we see in a dog infected with hepatozoon?
Hint it is NOT nodules in the lungs because we are NOT in the fungal section. |
Periosteal proliferation, prob in the hindlimbs
Were gonna use triple therapy x2 weeks to treat hepatozoon. Whats that?
TMS, clinda, pyrimethamine
Whats the pig coccidiostat a dog with hepatozoon has to be on for the rest of his life?
Decoquinate
What tick dz can you do a skeletal muscle bx for diagnosis?
Hepatozoon
