Nephrotoxic drugs
NSAIDS
Diuretics
Aminoglucosides
Cyclosporin
Gold
(Nephrotoxic Drugs Are Commonly Given)
Acetazolamide
Carbonic anhydrase inhibitor causing NaHCO3 diuresis and reduction in total HCO3 stores

Tx: glaucoma, urinary alkalinization, metabolic alkalosis, altitude sickness

ae: hyperchloremic metabolic acidosis, neuropathy, NH3 toxicity, sulfa allergy
(ACIDazolamide causes ACIDosis)
Furosemide
Sulfonamide loop diuretic

Inhibits cotransport system (Na, K, 2Cl) ot TAL

Tx: edemedous states

AE: ototoxicity, hypokalemia, dehydration, allergy, nephritis (interstitial), gout
(OH DANG)
Ethacrynic acid
Not a sulfonamide, same action as furosemide
Hydrochlorothiazide
Inhibits NaCl reabsorption in early distal tubule, reducing diluting capacity of the nephron. Decreased Ca excretion.

Tx: HTN, CHF, idiopathic hypercalciuria, nephrogenic diabetes insipidus

AE: hypokalemic metabolic alkalosis, hyponatremia, hyperglycemia, hyperlipidemia, hyperuricemia, and hypercalcemia, sulfa allergy
Spironolactone
K sparing diuretic

Competative aldosterone receptor antagonist in the cortical collecting tubule

Tx: hyperaldosteronism, K depletion, CHF

AE: hyperkalemia (leading to arrhythmias), gynecomastia, antiandrogen effects
Triamterene
Block Na channels in the cortical collecting tubule
Tx: hyperaldosteronism, K depletion, CHF
AE: hyperkalemia
Amiloride
Block Na channels in the cortical collecting tubule
Tx: hyperaldosteronism, K depletion, CHF
AE: hyperkalemia
Captopril, enalapril, lisinopril
ACE inhibitors, renin release is increased due to loss of feedback inhibition

Tx: HTN, CHF, diabetic renal disease

AE: cough, angioedemia, proteinuria, taste changes, hypOtension, pregnancy problems (fetal renal damage), rash, increased renin, lower angiotensin II

(CAPTOPRIL)

avoid with bilateral renal artery stenosis
Losartan
angiotensin II receptor antagonist, doesn't cause cough or angioedema
Drugs that cause gynecomastia
spirololactone, digitalis, cimetidine, alcohol, ketoconazole
(Some Drugs Create Awesome Knockers)
