Confused-Agitated: behavior is bizarre and non-purposeful relative to immediate environment, unable to cooperate directly with treatment efforts, verbalization incoherent or inappropriate, selective attention often nonexistent, pt. lacks short and long term recall

Confused inappropriate: unable to respond to simple commands consistently, shows gross attention to environment but highly distractible and lacks ability to focus on task , verbalization inappropriate, memory severely impaired with inappropriate use of objects, may perform previously learned tasks but unable to perform new ones, with structure may converse on social automatic level for short periods of time

Confused Appropriate: pt shows goal directed behavior but is dependent on external input or direction, follows simple directions, shows carryover for relearned tasks, responses may be incorrect due to memory problems but are appropriate to the situation, past memories show more depth than recent memory

Automatic Appropriate: appears appropriate and oriented within hospital and home settings, goes through daily routine automatically but robot-like, has shallow recall of activities with minimal/no confusion, shows recall for new learning but at a decreased rate, within structure can initiate social/recreational activities, judgement remains impaired

Purposeful Appropriate: able to recall and integrate past and recent events, aware and responsive to environment, shows carryover for new learning and needs no supervision once activities are learned, may continue to show decreased ability relative to premorbid abilities, reasoning, stress, and judgement in emergencies or unusual circumstances
