Level I
NO RESPONSE- TOTAL ASSIST
-Complete absence of observable change in behavior when presented visual, auditory, tactile, proprioceptive, vestibular or painful stimuli.
Level II
GENERALIZED RESPONSE- TOTAL ASSIST
- Demonstrates generalized reflex response to painful stimuli
- Responses may be significantly delayed
- Responds to repeated auditory stimuli with increased or decreased activity
- Responds to external stimuli with physiological changes generalized, gross body movement and/or not purposeful vocalization
Level III
LOCALIZED RESPONSE- TOTAL ASSIST
- Demonstrates withdrawl or vocalization to painful stimuli
- Turns toward or away from auditory stimuli
- Blinks when strong light crosses visual field
- Responds inconsistently to simple commands
- May responds to some persons but not others
Level IV
CONFUSED/AGITATED- MAX ASSIST
- Alert and in heightened state of activity
- Purposeful attempts to remove restraints or tubes, crawl out of bed
- May perform motor activities such as sitting or walking, but without apparent purpose or at request.
- Absent short term memory
- May exhibit aggressive or flight behavior
- Mood swings without relationship to environmental events

Level V
CONFUSED, INAPPROPRIATE NON-AGITATED- MAX ASSIST
- Alert, no-agitated but may wander randomly or in attempt to go home
- May become agitated in response to external stimulation
- Not oriented to person, place or time
- Frequent brief periods, non-purposeful sustained attention
- Severely impaired recent memory, confusion of past and present
- Absent goal directed, problem solving
- Inappropriate use of objects without external direction
- Unable to learn new information, but may be able to perform previously learned tasks with structure/cues
- Able to respond appropriately to simple commands with cues
- Able to converse on social automatic level with cues
Level VI
CONFUSED, APPROPRIATE- MOD ASSIST
- Inconsistently oriented to person, time and place
- Able to attend to highly familiar tasks in non-distracting environment for 30 min with moderate redirection
- Remote memory has more detail than recent memory
- Vague recognition of some staff
- Moderate assist to problem solve barriers to task completion
- Supervised for old learning (Self care)
- Shows carry-over for relearned familiar tasks (self care)
- Max assist for new learning with little or no carry-over
- Unaware of disabilities, impairments, safety risks
- Verbal expressions are appropriate in highly familiar and structured situations
Level VII
AUTOMATIC, APPROPRIATE- MIN ASSIST FOR DAILY LIVING SKILLS
- Consistently oriented to person and place within familiar environments. Mod assist for orientation to time.
- Able to attend to highly familiar tasks in non distraction environment with min assist to complete tasks
- Min supervision for new learning
- Demonstrates carry-over of new learning
- Monitors accuracy and completeness of each step in routine personal and household ADL and modify plan with min assistance.
- Superficial awareness of condition, but unaware of specific impairments
- Min supervision for safety in routine home and community activities

Level VIII
PURPOSEFUL, APPROPRIATE- STAND-BY ASSISTANCE
- Consistently oriented to person, place and time
- Independently completes familiar tasks for 1 hour (distracting environment)
- Uses assistive memory devices with stand-by assistance
- Initiates steps to familiar routines with stand-by assistance; modifies plan with min assistance.
- Requires no assistance once new tasks are learned
- Acknowledges impairments when they interfere with task completion, but requires stand-by assistance for corrective action
- Thinks about consequences of a decision with minimal assistance
- Over/under estimates abilities
- Depressed, irritable, easily angered, argumentative, self-centered
- Able to recognize and acknowledge inappropriate social behavior and takes corrective action with min assistance.
Level IX
PURPOSEFUL,APPROPRIATE- STAND BY ASSISTANCE ON REQUEST
- Independently shifts between tasks and completes them accurately for at least 2 hours.
- Uses assistive memory devices with assistance when requested
- Initiates and carries out steps to unfamiliar tasks with req assist.
- Aware of impairments, takes appropriate corrective action, but requires stand-by assist to anticipate problem before it occurs and action to avoid it.
- Acknowledges others' needs and feelings and responds appropriately with stand-by assistance.
- Depression may continue
- May be easily irritable and/or have low frustration tolerance.
Level X
PURPOSEFUL, APPROPRIATE- MOD INDEPENDENT
- Able to handle multiple tasks in all environments but may require breaks
- Independently procures and uses assistive memory devices
- Independently carries out steps to unfamiliar tasks, but may require additional time or compensatory strategies for appropriate decision/action
- Independently thinks about consequences, but may require additional time or compensatory strategies to select appropriate action
- Accurately estimates abilities and adjusts to task demands
- Able to recognize needs and feelings of others and responds appropriately
- Periodic depression may occur
- Irritable/low frustration tolerance when sick, fatigues, or stressed
- Social interaction behavior consistently appropriate
