What nerve is tested when you cover one eye. Have patient look straight ahead and advise when they see the finger of the examiner in the peripheral of the patient. Shine light. Do both eyes. Start at edge of peripheral for examiner.
Optic Nerve
What are the abnormal signs of an optic nerve dysfuntion?
If optic nerve is completely interrupted, patient is ipsilaterally blind; response slow or absent with light
Name the nerve being tested: Examine distance b/w upper and lower eyelids and pupil relative to them. Have patient look up w/o moving the head
Occulomotor III
Name the nerve being tested: Patient follows finger to 50° add and then down
Trochlear IV
Name the nerve being tested: Close patient’s eyes, touch and pin light touch to asses forehead, cheek and chin sensation; touch outer cornea; MMT jaw; reflex on chin
Trigeminal V
Name the nerve being tested: Observe eyes when looking forward and have them follow finger laterally
Abducens VI
Name the nerve being tested: Facial movements: raise eyebrows,, close eyes, smile, puff cheeks
Facial VII
Name the nerve being tested: Auditory function: rub fingers together near patient’s ear, use tuning fork near mastoid process
Vestibulocochlear VIII
Name the nerve being tested: Touch soft palate with cotton swab
Glossopharyngeal IX
Name the nerve being tested: Examine palate when patient says “ah”
Vagus X
Name the nerve being tested: MMT: SCM and upper trap
Accessory XI
Name the nerve being tested: Patient protrude tongue; examiner pushes tongue to one side

What are abnormal signs of the occulomotor nerve?
Abnormal: height of distance is abnormal and drooping eyelid doesn’t retract with upward gaze; indicates occulomotor nerve lesion; also seen with dilated pupil, later and downward deviation of eye
What are abnormal signs of the trochlear nerve?
Abnormal: Can’t look inferomedially, reports double vision and difficulty reading, descending stairs
What are abnormal signs of the trigeminal nerve?
Abnormal: anesthesia in affected area or severe pain; eyes don’t close; jaw deviates to weak side; lost or decreased reflex
What are abnormal signs of the abducens nerve?
Abnormal: one eye looks inward and double vision and deficit in abduction
What are abnormal signs of the facial nerve?
Abnormal: paralysis or paresis with upper and lower face equally involved (Bell’s palsy)
What are abnormal signs of the vestibulocochlear nerve?
Abnormal: difference in acuity; patient can hear through air but not through bone
What are abnormal signs of the glossopharyngeal nerve?
Abnormal: lack of gag reflex or asymmetrical elevation of soft palat
What are abnormal signs of the vagus nerve?
Asymmetrical elevation of soft palate, hoarseness
What are abnormal signs of the accessory nerve?
Abnormal: paralysis or paresis
What are abnormal signs of the hypoglossal nerve?
Abnormal: tongue deviates to one side; force of tongue overcome easily by examiner
