Characteristics of tardive dyskinesia as caused by antipsychotics
Tardive dyskinesia occurs within months to years of treatment. Patients have perioral movements (darting, protruding movements of the tongue, chewing, grimacing, puckering), and choreoathetoid movements of the head, limbs, and trunk.
Treatment for tardive dyskinesia caused by antipsychotics
There is no known treatment. Discontinue the drug. Clozapine may help.
Characteristics of neuroleptic malignant syndrome as caused by antipsychotics
Neuroleptic malignant syndrome can occur at any time during treatment. It is a life-threatening condtion characterized by rigidity, mutism, obtundation, sweating, and agitation.
Laboratory presentation of neuroleptic malignant syndrome
- high fever (107 degrees)
- high CPK (often >5000)
- myoglobinuria
Treatment for neuroleptic malignant syndrome
1. Discontinue the drug
2. Supportive care
3. Dantrolene (as in malignant hyperthermia) or bromocriptine (restores dopamine activity)
