What are the various etiologies of a chronic daily headache?
Migraine, medication overuse, tension headache, cluster headache, hemicrania, SUNCT
What are the temporal considerations of a chronic daily headache?
greater than 4 hours in duration, more than 15 days per month, more than 3 months, without organic cause.

TCA, amytriptiline or nortryptiline, tapered from 10mg up by 10mg. Alternatively, SNRIs work well. Relaxation and CBT should be attempted.
