permanent Sxs of CMV?
Hearing loss
Seizures
What separates CMV from EBV?
Mono is heterophile negative
Transmission of CMV (5)
droplets
fecal oral
transplacental
breast milk
STD
Diagnosis of CMV?
IgM antibody on TORCH panel
Buffy Coat

Histo: Owl's Eyes
Pathology of CMV?
Affects lymphocytes, lies latent in killer cells

Stops MHC I and MHC II
secretes IL-10, decreases TH1 response, decreases CMI
What are the Sxs of CMV in an infant?
Small head, erethematous rash, icterus, hepatosplenomegaly, sensorineural hearing loss

Imaging: periventricular calcifications

Mullberry muffin rash
HIV patient with a seizure, recent history of severe headache, lymphadenopathy, papillidema, and loves cats
Toxoplasmosis
cresent-shapped trophozoite within macrophages
toxoplasmosis
cysts ingested from undercooked meat or from cats
toxoplasmosis
pathogenesis of toxoplasmosis
cysts ingested --> small intestine and release invasive form which penetrate intestinal wall and then disseminate or get taken up by macrophages

host response contains the infection in tissue, and invasive form becomes dormant. If host becomes immuno compromised --> encephalitis
Microcephaly, intracerebral calcification, chorioretinitis and mental retardation in a kid
congenital toxoplasmosis
Nodular enhancements or Ring mass lesion on MRI
toxoplasmosis
What is the MCC of encephalitis in HIV?
Toxoplasmosis
IgB, IgD spikes
glycoprotein coat
Nucleocapsid
DS DNA core
Herpes simplex virus
25 year old homosexual male with painful skin rash in the perianal area, hx of unprotected sex with multiple partners, vesicular rash in clusters on erethematous base
herpes
pathogenesis of herpes
primary infection goes to epithelial cells, causes lesions, enters nerves, traves retrograde and uncoats and tethers itself there. Reactivates in S2-S5 dermatome
what causes reactivation of herpes?
stress, disease

when the neuron becomes metabolically active, the HSV activates
Why cant the body take care of HSV primary infections?
it moves into neurons so fast it avoids the primary immune response on initial infection.

The body will make antibody and this lessens the recurrent infections
how long do sxs last in HSV?

How long does the virus shed in HSV?
symptoms last from 12-20 days

Shedding lasts 12 days and starts 2 days before lesions show
neonatal herpes, 3 contidions
1) SEM: skin, eyes, mouth

Labs show CSF with decreased glucose,
Listeria Monocytogenes
2 month old with fever, convulsions, irritability, poor eating, widespread rash and stiff neck
Listeria Monocytogenes
Pathobiology of Listeria Monocytogenes
heat resistant listeria survive poor pasturization --> GI --> phagocytes --> blood --> neural tissue --> meningitis
The only gram positive with endotoxin
Listeria Monocytogenes
Why should preggos avoid cold cuts and soft cheeses?
Listeria Monocytogenes
PDA, cataracts, deafness, and blueberry muffin rash. mom had flu like sxs in 1st trimester
