Suboxone
-Buprenorphine and naloxone
-Physcian needs a unique ID # begining with X
-DEA require that this ID number and the exsisting DEA registration numberr be inccluded on all perscriptions for Suboxone for the treatment of opiod dependence.

-Cannot be refilled
-Expire within 1 year

-Pharmacist can never make changes to patient's name, narcotic prescibed except generic substitution, or th perscriber's signature. (WIll need new RX)
-For other changes always document time and date the perscriber was coontacted and always ask the practioner to document the change in the patient's chart.
-A fax may serve as an original C-II RX only if 1)rx for substance compounded for direct administration. 2) Rx for a LTCF patient. 3)Rx for a Hospice and or paid by Medicare.

-expire in 6mths
-has only 5 Refills ( 6 fills in total)

-must be dispensed for medical purpose
-pharmacist can perscribe
Criteria to question a controlled Rx
1) Frequent loss of drugs
2) Only controls are prescribed to pts
3)One person presents Rx with several names
4)Same/similar controls prescribed by 2+ prescribers.

If suspect a Rph is involved in diversion of controls report them to Dept. of Health.
C lll to C V
-Partial filling is permitted:
1) each partial filling is recorded as a refill
2)Total quantity dispensed in all partail fillings does not exceed the total quantity prescribed.
3) no dispensing occurs after 6 mths after the date on the RX issued.
