Dopamine

Trade Name: Intropin




Class: Adrenergic agonist, inotropic, vasopressor




MOA: Stimulates α and β adrenergic receptor, Moderate doses = inotropy (increase force) and increase cardiac output maintaining vasodilation. HIgh doses = vasoconstriction and peripheral vascular resistance




Ind: Hypotension and decreased cardiac output with cardiogenic and septic shock, HoTN after cardiac arrest




Adv: Tach, arrhythmias, soft tissue necrosis, HTN, angina

Cont: VF, VT, V arrhythmias; fix hypovolemia before administering

Epinepherine

Trade Name: EpiPen




MOA: binds α and β receptors, increases BP, increased heart rate, bronchodilation




Ind: Bronchospasm, allergic/anaphylactic reaction, restored cardiac activity in cardiac arrest




Adv: Anxiety, cardiac arrhythmias, HoTN, chest pain, tremors




Cont: Arrhythmias (other than VF), asystole, cardiac disease, HTN, diabetes

Norepinepherine

Class: Adrenergic agonist, inotropic, vasopressor




MOA: α1, α2, β1 agonist, peripheral vasoconstriction, increase BP, cardiac inotropy, dilation of coronary arteries




Ind: Cardiogenic shock, septic shock, severe HoTN




Adv: Dizziness, anxiety, cardiac arrhythmias, dyspnea, exacerbated asthma




Cont: MAOI (anti depressents); take caution in hypovolemia

Dobutamine

Class: Adrenergic, inotropic




MOA: Increase cardiac contractility and stroke volume, increase cardiac output




Ind: CHF, cardiogenic shock




Adv: Tach, PVCs, hypertension, hypotension, palpations, arrhythmias




Cont: Poison/drug induced shock, sys BP <100 w/ signs of shock, idiopathic hypertrophic subaortic stenosis

Atropine

Class: Anticholinergic




MOA: Competes reversibly with ACh @ muscarnic receptor.


Receptors affected: salivary, bronchial, sweat glands, eyes, cardiac, GI tract




Ind: Symptomatic bradycardia, asystole, organophosphate poisoning




Adv: decreased secretions (dry mouth, hot skin), facial flushing, blurred vision, tachycardia, paradoxical bradycardia (if too low dose admin too slow)




Cont: Acute MI; GI obstruction; closed angle glaucoma

Metoprolol

Trade Name: Lopressor




Class: β adrenergic antagonist, antianginal, antihypertensive, class II antiarrhythmic




MOA: Inhibits inotropy as well as cardiac rate; results in decrease of cardiac oxygen consumption




Ind: ACS, HTN, SVT, atrial flutter, AF, thyrotoxicosis




Adv: Tiredness, dizziness, diarrhea, cardiac block, bradycardia, bronchospasm, drop in BP




Cont: cardiogenic shock, AV block, bradycardia

Class: bronchodilator, β agonist




MOA: Binds and stimulates β2 receptors; relaxation of bronchial smooth muscle




Ind: asthma, bronchitis w/ bronchospasms, COPD




Adv: hyperglycemia, hypoalemia, palpitations, sinus tach, anxiety, tremor, nausea, throat irritation, dry mouth, HTN, dyspepsia, insomnia, headaches, epistaxis, paradoxical bronchospasm
