Prostaglandins function
can cause smooth muscle contraction or relaxation;
inhibit gastric secretion;
PGE cause pain when injected intradermally

INDUCE LABOR
RENAL SALT & WATER EXCRETION ; increases renal blood flow

Can vasodialate or vasoconstrict
A term used to point out a characteristic of a person. (Swift-footed Achilles) Can be abusive, or offensive, but are not so by definition. (The Rock, Jake "the Snake")
epithet
Prostacyclin fxn
Inhibits platelet aggregation
Vasodialates
INHIBITS GASTRIC SECRETION

Sensitive to pain
Leukotriene fxn
CAUSE HYPOTENSION;
Cause anaphylaxis (s.m. constriction)
Bronchoconstriction
Chemotactic
When are Prostaglandins formed?
Under inflammatory conditions causing pain and vasodialation
Aspirin MOA
Aspirin irreversibly and non-selectively inhibits fatty acid cyclo-genases,
Apsirin properties
antipyretic
Analgesia (blocks pain)
Anti-inflammatory
Uncouples oxidative phosphorylation
FDA indications for aspirin
. Cerebrovascular accident; prophylaxis b. Fever c. Generalized aches and pains d. Headache e. Migraine f. Myocardial infarction g. Myocardia infarction; Prophylaxis h. Osteoarthritis i. Pain j. Rheumatoid arthritis
Aspirin Adverse effects
1) Gastric Irritant (blocks prostacyclin & prostaglandin)

2)Hypersensitivity --> anaphylaxis (b/c pushing toward more leukotrienes??)
--> nasal polyps = risk factor
Aspirin toxicity
Salicylism and RENAL toxicity?
What is Substituted Acetic Acid?
Indomethacin - one of the most potent inhibitors of fatty acid cyclo-oxygenase thus
inhibiting prostaglandin, thromboxane and prostacyclin synthesis.
FDA uses for Indomethacin
1) Ankylosing spondylitis*
2) Gouty arthritis, acute*
3) Osteoarthritis
4) Pain
5) Patent ductus arteriosis*
6) Barter's syndrome
5) RA*
Toxicity of Indomethacin
gastro-intestinal complaints hypersensitivity - usually shows cross-reactivity with aspirin CNS effects - headache, vertigo, somnolence
What are propionic acid deritvatives?
1) Ibuprofen
2) Naproxen

The proprionic acid derivatives are usually better tolerated than aspirin, indomethacin and the pyranzolones.

They are inhibitors of cyclo-oxygenase. They can produce aspirin- like hypersensitivity.
Ketorolac Tromethamine

Pharmokinetics?
1) injection
2) plasma bound protien
3) TRANSPLACENTAL!!! NOT FOR PREGNANT WOMEN!!!!
4) RENAL CLEARANCE
FDA label indications for Ketorolac
1) Extraction of cataract-Inflammatory disorder of the eye
2) Pain, Short term
3) Pain in eye- Refractive keratoplasty
4)Seasonal allergic conjunctivitis
MOA of Celecoxib?
Selective for COX II
PHARMACOKINETICS for Celecoxib
a. absorption decreased by food b. effective t1/2 = 11 hours
c. high plasma protein binding d. metabolized by CYP 2C9
