Severe pre-eclampsia, HELLP and eclampsia
-Severe preeclampsia with hypercoagulability and vasoconstriction, hemolysis, elevated liver enzymes, low platelets
-Tx: stabilize mom
deliver fetus
magnesium sulfate to prevent seizures
lower BP with labetolol or hydralazine
preeclampsia with new onset tonic clonic seizures
-MgSo4 IV bolus then infusion
-measure serum levels
-MONITOR DTRs and respiratory status
magnesium toxicity
-loss of patellar reflex, flushing and warmth

antiresorptive meds

major first line agents for osteoporosis
1. Bisphosphates:
lendronate, risendronate (po qAM, or once weekly), aldendronate (Po qAM)
-some IV q3months
AE of bisphosphates
-take with 8oz water on empty stomach
-N/ dyspepsia, esophageal erosion, muscle aches
-DO NOT use in pts with esophageal strictures or achalsia
-jaw damage (mostly in cancer pts using IV form-Zometa)
Calcitonin
-tx of osteoporosis when estrogen or other therapies are CI or not tolerated
-intranasal or injection
-AE: rhinitis, epistaxis, local inflamm, N/V
Teriparatide (Forteo)
-parathyroid hormone given subcut
-for osteoporosis with high risk factors
-AE: N, constipation, leg cramps, dizziness, depression
-may act on osteoblasts to stimulate new bone growth
raloxifene (Evista)
-oral tablet for osteoporosis
-prevention and treatment
Levonorgestrel
-plan B
-a progestin
-taken once within 72hrs of unprotected intercourse to PREVENT PREGNANCY
Menses should occur in 3 weeks otherwise need to do a pregnancy test.
