Calcium Channel Blockers AKA?
The "PINES" wks on the pipes
What are the indications for CCB?
HTN, Variant angina and Dysrhythmia
What is the MOA of CCB?
Inhibits Ca ions into smooth & cardiac muscle.
Therefore it.... Dilates arterioles
Decrease PVR
Decreases contractility
Decreases conduction
What is the T/E of CCB?
Decrease BP
What are the S/E of CCB?
Palpitations, reflex tachycardia, arrhythmia's, hypotension, HF, dizziness, edema, HA, constipation
What are C/C of CCB?
Certain dysthythmias
CCB are selective and non selective. What do the selective target? What do the non-selective target?
Selective - Blood vessels only
Non-selective - heart and blood vessels = vasodilation
What are N/I for CCB?
Monitor BP, HR, EKG, orthostatic hypotension, S/S HF, Weight gain, CP, can cause reflex tachycardia d/t rapid decrease BP
What are pt teaching's for CCB?
Report dyspnea, CP, decrease Hr, palpitations, wt gain, avoid grapefruit juice can increase CCB levels, etoh can increase orthostatic hypotension, do not stop taking suddenly
Cardiac glycoside
Digoxin
What is the antidote for Digoxin?
Digibind
What are the indications for C(g)?
HF and dysrhythmia
What is the MOA of C(g)?
Increase cardiac contractility by increasing Ca ions into myocardial cells and decreasing conduction
What is the T/E of C(g)?
Increase force of contraction (+ inotropic effect)
Decrease HR
What are S/E of C(g)?
CV: Bradycardia, Arrhymias, Fatigue
CNS: Drowsiness, dizzy, visual disturb (halo's)
GI: Anorexia, n/v
What are C/C in C(g)?
Caution: Renal Failure, elderly, acute MI
Contraindication: Ventricular dysrhythmias
NARROW THERAPEUTIC INDEX!!!!!!!!
What are S/S of toxicity w/ C(g)?
N/V, HR <60, blurred vision or greenish/ yellow tinges around objects
