3 drugs that inhibit platelet aggregatation?
aspirin (@COX1)
clopidogrel/ticlopene (@ADP)
Abciximab/Integrelin (@intengrin)
Only inhibits factor X?
LMWH (enoxaparin, dalteparin)
enoxaparin?
LMWH
Inhibits factor X as well as activating ATIII?
Heparin
Mechanism of heparin
Primarily activated ATIII to inhibit factors II, VII, IX, and X.

Secondarily, directly inhibits factor X
Competitive antagonist of vit K reductase?
warfarin/coumadin = TERATOGENIC
Oral anticoagulant?
warfarin/coumadin
Fibrinolytic drugs?
"clot busters"
tPA
streptokinase

plasminogen -->plasmin
Do nitrates decrease preload, afterload, or neither?
They decrease both via veno and arterio dilation

Side effects: reflex bradycardia
TOLERANCE (couple hours)
Treatment for stable angina?
Nitrates
Treatment for unstable angina?
Ca channel blockers:
diltiazam
verapamil

Beta-blockers
Mechanism of Ca channel blockers?
vasodilation (smooth muscle relaxation)
neg ionotropes
neg chronotropes
What type of angina do you treat with dihydropyridine?
Prinzmetal's

Dihydropyridine only acts as a vasodilator
How do bile acid sequestrants work?
They (cholestyramine) bind bile in the gut preventing reabsorption. The liver inc production of bile, creating demand for cholesterol. To meet this demand there are inc ldl receptors on the liver, which clear the ldl from the plasma. DECREASES LDL.

Also inc TG.
Should you use cholestyramine in someone who has hypertriglyceremia?
NO, bile acid sequestrants (cholestryramine) inc TG's.
Drug that increases HDL?
Niacin
HMG-CoA reductase?
Statins
Drug of choice to decrease LDL?
Statins
Used to decrease TG?
Gemfibrozil
Tx for HTN in a diabetic?
Captopril
Side effects of ACE i's?
cough, rash, dry skin...mediated via brakykin
Difference between ACEi's and ARB's?
Besides mechanism, arb's have less side effects i.e NO cough
Classes of antiarrhythmics?
Class I: Na blockers
A = mod Na block
B = mild Na block
C = strong Na block
Class II: Beta Blockers
Class III: K blockers
Class IV: Ca Channel blockers
Adenosine
Class I anti-arrhythmics?
IA = quinidine, procainamide
IB = lidocaine
IC = flecanide = NEVER USE
What is the effect of beta-blockers in terms of anti-arrhythmics?
Decrease AV nodal conductance.

Use for A-fib
#1 drug for a-fib/flutter? Drawback?
Amiodarone (Class I, II, III, IV, primarily class III)

Tons of side effects.
Tx for paroxsysmal supraventricular tachycardia?
Adenosine
What are two targets of treating angina?
Decrease O2 demand
Increase O2 supply
Inhibit's Na/K ATPase to increase ionotropy?
Digoxin/Digitalis
Ways to treat CHF?
Increase ionotropy
Decrease afterload
Drugs that decrease mortality when treating CHF?
B-blockers, ACE i's/ARB's, Spironalactone
Tx CHF by inc ionotropy?
Digitalis/Digoxin
Symptomatic improvement in CHF?
diuretics
digoxin
nitrates
