Cardiac Glycosides: Digoxin

A: inhibts sodium-potassium pump -> increases calcium -> increases myocardial contraction
-> increases cardiac output, which slows HR + increases contraction U: HF , atrial fib + flutter SE: bradycardia , arrthymias, heart block, nausea, vomit, diarrhea, anorexia , head ache , dizzy, drowsy CI: hypokalemia , greater risk of tox.. in renal failure + elderly , ventricular fib or tachycardia, heart block , acute MI , electrolyte abnormalities NI: give loading does w/ doct. then maintence doses orally , gradually increase dose until s/s dimish , give at same time each day, if pulse is <60 or >120 hold dose + notify doct. , report gain of 5 lbs or more |

U: acute attacks , angina SE: hypotension , syncope , dizzy CI: hypotension, anemia , hypovolemia, intracranical pressure , Levitra/Viargra -> sever life threatening hypotension, alcohol use will lower BP NI: give 1 spray every 5 min - max of 3 over 15 min. , store in org. container from light/heat/moisture , replace every 6 months regardless of exp. date, should feel tingle , remove patches at night |
