What is peripheral arterial disease
Vascular insufficiencies caused by noncoronary arteries secondary to atherosclerotic occlussions

Symptoms:
Leg or hip pain, cold legs and feet, changes in skin color, pain that is reduced upon resting, numbness or tingling
What risk factors exist for PAD
Age > 50
Cigarette smoker
Diabetes
Cholesterol
Family hx
HTN
Male sex
Homocysteine elevated
What is the drug treatment for PAD
1) Smoking cessation - most important!
2) Statins - goal LDL <70
3) Fibric acid agents - used with high TG
4) Antihypertensives - goal BP of less than 140/90, <130 if has CKD or DM
5) Antiplatelet agents for all patients. Aspirin 75-325mg
How do you treat claudication
1) Cilostazol (Pletal)
Phosphodiesterase inhibitor that is vasodilatory and antiplatelet
Avoid use in HF

2) Pentoxyifylline (Trentall)
Reduces blood and plasma viscosity, inhibits neutrophil adhesion and activation
Second line agent
How do you treat critical limb ischemia
1) IV pentoxyifylline
2) Oral vasodilator prostaglandins
3) Endovascular treatment
