What is the first step in responding?

BSI(Body Substance Isolation)- Is the scene safe?

What are the six main steps of Patient Assessment/management-trauma

1.Scene size up

2. Primary survey/resuscitation

3. History taking

4. Secondary assessment

5. Vital Signs

6. Reassessment

SCENE SIZE UP

1.Determines the scene/situation is safe.

2. Determines the mechanism of injury/nature of illness.

3. Determines the number of patients.

4. Request additional EMS assistance if necessary.

5. Considers stabilization of the spine.

PRIMARY SURVEY/RESUSCITATION

1. Verbalizes general impression of the patient.

2. Determines responsiveness/level of consciousness.

3. Determines chief complaint/apparent life threats.

4. Airway

5. Breathing

6. Circulation

7. Identifies patient priority and makes treatment/ transport decision(based on calculated GCS scale)

PRIMARY SERVEY/RESUSCITATION- BREATHING

1. Assesses breathing

2. Assures adequate ventilation

3.Initiates appropriate oxygen therapy

4. Manages any injury which may compromise breathing/ ventilation

PRIMARY SERVEY/RESUSCITATION- CIRCULATION

1. Checks pulse

2. Assesses skin[skin color, temperature or condition.]

3. Assesses for and controls major bleeding if present.

4. Initiates shock management [positions patient properly, conserves body heat]

HISTORY TAKING

Attempts to obtain sample history.

Signs and Symptoms

Allergies

Medications

Pertinent past medical history

Last oral intake

Events leading up to injury or illness

SECONDARY ASSESSMENT

1. Head

2. Neck

3. Chest

4. Abdomen/pelvis

5. Lower extremities

6. Upper extremities

7. Posterior thorax, lumbar, buttocks

SECONDARY ASSESSMENT/HEAD

1. Inspects mouth, nose, and assesses facial area

2. Inspects and palpates scalp and ears

3. Assesses eyes

SECONDARY ASSESSMENT/NECK

1. Checks position of trachea

2. Checks jugular veins

3. Palpates cervical spine

SECONDARY ASSESSMENT/CHEST

1. Inspects chest

2. Palpates chest

3. Auscultates chest

SECONDARY ASSESSMENT/ABDOMEN-PELVIS

1. Inspects and palpates abdomen

2. Assesses pelvis

3. Verbalizes assessment of genitalia/perineum as needed.

SECONDARY ASSESSMENT/LOWER EXTEMITIES

1. Inspects, palpates and assesses motor, sensory and distal circulatory functions

SECONDARY ASSESSMENT/UPPER EXTEMITIES

1. Inspects, palpates and assesses motor, sensory and distal circulatory functions.

SECONDARY ASSESSMENT/POSTERIOR THORAX, LUMBAR, AND BUTTOCKS

1. Inspects and palpates posterior thorax

2. Inspects and palpates lumbar and buttocks areas

VITAL SIGNS

1. Obtains baseline vital signs[Must include, BP,P, R]

2. Manages secondary injuries and wounds appropriately.

REASSESSMENT

1. Demonstrates how and when to reassess the patient.

CRITICAL CRITERIA

1. Failure to take or verbalize BSI precautions

2. Failure to initiate or call for transport of the patient with in 10 minute time limit.

3. Failure to determine scene safety.

4. Failure to assess for and provide spinal protection when indicated.

5. Failure to voice and ultimately provide high concentration of oxygen.

6. Failure to assess /provide adequate ventilation.

7. Failure to find or appropriately manage problems associated with airway, breathing, hemorrhage or shock

8. Failure to differentiate patient's need for immediate transport versus continued assessment/treatment at the scene.

9. Performs other assessment before assessing/treating threats to airway, breathing and circulation.

10. Uses or orders a dangerous or inappropriate intervention
