MAB drugs
Mifepristone (progesterone receptor antagonist - detachment of blastocyst) OR Methotrexate (antifolate drug - feticidal action on blastocyst)

Misoprostol - PGE1 - contracts uterus and expels contents

AE - N/D, fever

COMPS - excessive uterine bleeding, rupture, incomplete AB
Tocolysis - delay labor
Antenatal glucocorticoids

Tocolytic drugs given for up to 48 hrs:
Beta 2 agonists:
Terbutaline - relaxation by binding beta 2 receptors
Ritodrine
CI - cardiac dz, poorly controlled hyperthyroidism/DM

Calcium Channel Blocker - Nifedipine - relax by decreasing intracellular calcium

NSAID - Indomethacin - inhibit COX - relax by decreasing prostaglandin synthesis in uterus
AE - to fetus - premature closure of ductus arteriosus
Induction and maintenance of labor
Misoprostol - PGE1
Dinoprostone - PGE2
--> help in ripening cervix and initiating labor; intravaginally

Oxytocin - augments uterine contractions
AE - water nitoxication due to interaction w/ ADH receptors, excessive uterine contractions, rupture of uterus
Postpartum Hemorrhage
Oxytocin, if doesn't stop bleeding, use Ergonovine IM (partial agonistic effects on 5HT2 to stop and alpha recepters -- sustained contractions to stop uterine bleeding)
CI - HTN, liver/kidney dz, presence of sepsis
Category A
no risk to human fetus
Category B
animal studies show no risk, but no studies in people
Category C
risks cannot be r/o
Category D
positive evidence of human fetal risk but potential benefits from drug use outweigh potential risk
Category X
CI in pregnancy; risks outweigh benefits
