The MAIN PARTS of BLS |
(1) Chest Compressions
(2) Airway (3) Breathing (4) Defibrillation *Team-Based CPR Simultaneous BLS is BETTER |
|
Simplified ADULT BLS Algorithm
WITH DEFIBRILLATOR |
(1) No RESPONSE; NO/NO NORMAL BREATHING (gasps)
(2) ACTIVATE Emergency Response System (ERS) (3) GET DEFIBRILLATOR (4) Check Rhythm/SHOCK if indicated (every 2 sec.) (5) Start CPR; PUSH HARD/FAST (30:2 Ratio) START C-A-B |
|
Simplified ADULT BLS Algorithm
W/O DEFIBRILLATOR |
(1) UNRESPONSIVE; NO/NO NORMAL BREATHING (gasps)
(2) ACTIVATE Emergency Response System (ERS) (3) CHECK Pulse (take@least 5 sec NO > than 10 sec.) (4) Start CPR; PUSH HARD/FAST (30:2 Ratio) START C-A-B |
|
TEAM-BASED CPR
|
**EACH MEMBER TAKES A STEP**
(1) Activate ERS (2) BEGIN CHEST COMPRESSIONS (3) RETRIEVE Bag-Mask, START Ventilations (4) RETRIEVE Defibrillator, PREPARE for Use |
|
AGONAL GASPS
|
**May be present the initial 5 min. of sudden CA**
APPEARANCE: Draws air in quickly/gasps; weak or forceful; RATE: May be delay between gasps; SOUND: May sound like snort, snore or groan; TYPE: Gasping is NOT normal breathing&sign of CA in unresponsive person |
|
STEP 1
ASSESSMENT of Scene and Safety |
(1) Ensure scene is safe for victim&yourself
(2) Tap shoulder, SHOUT "ARE YOU ALL RIGHT?" (3) CHECK for Breathing, if NOT, ACTIVATE ERS |
|
STEP 2
ACTIVATE ERS, OBTAIN an AED |
(1) If ALONE w/UNRESPONSIVE victim w/o BREATHING, SHOUT for HELP
(2) Activate ERS (3) GET AED or DEFIBRILLATOR (4) CHECK Pulse (No < 5 sec. No > 10 sec.) (5) BEGIN CPR, Use C-A-B Sequence |
|
STEP 3
PULSE CHECK |
(1) LOCATE Trachea, PLACE 2or3 Fingers
(2) SLIDE 2or3 Fingers from TRACHEA grove 2 Muscles on Side of Neck, Feeling for CAROTID PULSE (3) FEEL Pulse for <5 sec. No >10 sec. (4) If NO PULSE, Begin CPR, STARTING w/Chest Compressions, C-A-B Sequence |
|
STEP 4
BEGIN CYCLES OF 30 CHEST COMPRESSIONS: 2 BREATHS (CPR) |
(1) USE Compression:Ventilation Ratio 30:2 (Press:Breath)
(2) PUSH HARD&FAST (3) RATE of 100/min. (4) ALLOW Chest 2 RECOIL COMPLETELY post compression (5) MINIMIZE INTERRUPTIONS in Compressions (6) REPEAT |
|
CHEST COMPRESSION TECHNIQUE
|
(1) POSITION at side of victim
(2) ENSURE victim lying FACE UP on FIRM, FLAT surface (keep HEAD/NECK/TORSO in line when positioning) (3) Place HEEL OF 1st HAND on CENTER of CHEST on the LOWER 1/2 of the BREASTBONE (4) Place HEEL OF 2nd HAND on TOP of 1st HAND or on WRIST of 1st Hand (5) STRAIGHTEN Arms, Shoulders OVER HANDS (6) PUSH HARD/FAST: Press@least 2 in. w/each compression, PRESS STRAIGHT DOWN n2 BREASTBONE (7) END of Compression, ALLOW Chest to RECOIL (8) MINIMIZE Interruptions |
|
OPENING AIRWAY FOR BREATHS:
Head Tilt-Chin Left |
(1) PLACE 1st Hand on FOREHEAD, PUSH w/PALM to TILT head BACK
(2) PLACE Fingers on 2nd Hand UNDER Bony Part of LOWER JAW Near the CHIN (3) LIFT Jaw to BRING CHIN FORWARD **DO NOT--Press Deeply into Soft Tissue as May BLOCK Airway; DO NOT Use THUMB to Lift the Chin; DO NOT Close Mouth COMPLETELY** |
|
GIVING ADULT: MOUTH TO MASK BREATHS
|
(1) POSITION self@victim SIDE
(2) PLACE mask on face, Use NOSE BRIDGE 4 guide (3) SEAL mask AGAINST face w/hand CLOSER 2 TOP of head, PLACE INDEX FINGER&THUMB along mask EDGE; PLACE THUMB of 2nd hand along mask BOTTOM (4) USE E-C Clamp Technique: a. PERFORM a HEAD TILT/CHIN LIFT b. PLACE mask w/narrow on Nose BRIDGE c. USE THUMB&INDEX finger of 1 hand 2 make a "C" on the SIDE of the mask, PRESSING EDGES of mask to face d. USE FINGERS to LIFT angles of jaw (3 fingers form "E"), OPEN airway&PRESS mask to face (4) DELIVER air OVER 1 sec. to make CHEST RISE |
|
OPENING THE AIRWAY FOR BREATHS:
JAW THRUST |
(1) PLACE 1 hand on each SIDE of head, resting elbows on surface where victim laying
(2) PLACE fingers UNDER angles of the LOWER jaw&LIFT w/BOTH hands, DISPLACING jaw FORWARD (3) IF LIPS CLOSE, PUSH lower lip w/THUMB 2 OPEN lips |
|
Foundational Facts:
DEFIBRILLATION |
(1) POWER ON AED (device will guide thru next steps)
(OPEN carrying case or AED top; TURN power ON (2) ATTACH AED PADS to BARE CHEST (Choose ADULT pads for ages 8+; PEEL backing from pads; ATTACH 1 ADHESIVE to UPPER-RIGHT chest-(below collarbone) OTHER on side of the LEFT NIPPLE w/TOP EDGE of pad a few inches from ARMPIT) (3) CLEAR the victim&ANALYZE the rhythm (4) IF SHOCK ADVISED, Shout "CLEAR" (no one touching victim), PRESS SHOCK button (5) IF NO SHOCK advised&AFTER DELIVERY of SHOCK; IMMEDIATELY RESUME CPR w/CHEST COMPRESSIONS (6) After 5 cycles or 2 min. of CPR; AED will PROMPT to REPEAT steps 3&4; If NO SHOCK advised, IMMEDIATELY RESTART CPR w/CHEST COMPRESSIONS **Time btwn last COMPRESSION&SHOCK to <10 sec.** |
|
SPECIAL SITUATIONS:
HAIRY--WATER--IMPLANTED DEFIBRILLATOR--TRANSDERMAL MEDICATION PATCH |
(1) HAIRY CHEST-Attach pads, if AED prompts "CHECK PADS" ensure adhesion to hair, pull pad off removing hair); SHAVE if large amounts of hair left; REPLACE pads.
(2) WATER-DO NOT USE AED in water; REMOVE victim from water; WIPE chest; FINE if snow/small puddle) (3) CHECK 4 Implanted Defibrillators; LUMPS (deck of cards) upper chest/abdomen; DO NOT SHOCK (4) TRANSDERMAL MEDICATION PATCH; DO NOT place pads ON TOP OF any medical patches |
