Contraindications: Active bleeding, recent CVA

Phenytoin (Dilantin)
Indications: Major motor seizures, status epilepticus

Contraindications: Sinus bradycardia, 2nd and 3rd block, SA block

Adverse Rxns: Hypotension w/ rapid push, dysrthymias, bradycardia, respiratory depression, ataxia, CNS depression

Dosage: 1000mg slow IV
Pediatric - 10-20mg/kg slow IV loading dose
Pancuronium (Pavulon)
Indications: Induction or maintenance of paralysis after intubation to assist ventilations

Contraindications: Inability to control airway, myasthenia gravis

Adverse Rxns: transient hypotension, tachycardia, dysrhythmias

Dosage: 0.04-0.1mg/kg slow IV
Pediatric - same
Infant - half dose
Norepinephrine (Levophed)
Indications: Cardiogenic shock, neurogenic shock, inotropic shock

Contraindications: Hypotension w/ hypovolemia

Adverse Rxns: Headache, dysrhythmias, tachycardia, HTN

Dosage: Dilute 4mg in 250mL of D5W; infuse at 0.5-5mcg/min - titrate to effect
Nitroglycerin
Indications: ischemic chest pain, CHF, HTN emergencies, pulmonary HTN

Contraindications: Hypotension, head injury, cerebral hemorrhage, erectile dysfunction meds

Adverse Rxns: Headache, syncope, hypotension

Dosage: 0.3-0.4mg SL
Albuterol
Indications: Bronchospasm

Contraindications: Heart dysrhythmias with tachycardia

Adverse Rxns: Dizziness, tachycardia, dysrhythmias

Dosage: Metered dose - 1-2 inhalations
Solution - 2.5mg diluted to 3mL w/ NS
Midazolam Hydrochloride (Versed)
Indications: Premedication for intubation or cardioversion, seizures in children

Contraindications: Shock, coma, alcohol intoxication, depressed vital signs

Adverse Rxns: Respiratory depression, hiccup, cough

Dosage: Sedation - 1-2.5mg slow IV
Elderly - 0.5mg slow IV
Pediatric - 0.05-0.2mg/kg then infuse
Seizures in Peds - 0.1-0.15mg/kg IV slow
Meperidine (Demerol)
Indications: Moderate to severe pain, preoperative medication

Contraindications: Head injury

Adverse Rxns: Respiratory depression, euphoria, agitation, seizure, hypotension, coma

Dosage: 50-100mg IM; 15-35mg IV
Elderly - 25mg IM
Pediatric - 1-2mg/kg
Mannitol (Osmitrol)
Indications: Cerebral edema, rhabdomyolysis

Contraindications: Severe hypotension, dehydration, active cranial hemorrage, hyponatremia

Adverse Rxns: Pulmonary edema, renal failure, CHF

Dosage: 0.5-1g/kg/dose IV infusion over 30-60 min
Hydralazine (Apresoline)
Indications: HTN emergencies - mainly of eclampsia and preeclampsia, pulmonary HTN

Contraindications: CAD, congestion

Adverse Rxns: Tachycardia, hypotension, headache

Dosage: 10-40mg IM or IV - repeat in 10min

Indications: Nausea/vomiting, anxiety rxns, motion sickness, pruritus

Contraindications: Hypersensitivity

Adverse Rxns: Dry mouth, drowsiness

Indications: Reversal of benzodiazepine sedation or overdose

Contraindications: Cocaine, cyclic antidepressant overdose

Epinephrine Racemic
Indications: Bronchial asthma, bronchospasm, croup, laryngeal edema

Contraindications: HTN, epiglotitis

Dosage: Metered - 2-3 inhalations
Solution - Dilute 5mL in 5mL saline, admin over 15 min
Isoproterenol (Isuprel)
Indications: Management of polymorphic V-Tach

Contraindications: Monomorphic Vtach, vfib, hypotension, cardiac arrest, angina

Adverse Rxns: Dysrhythmias, hypotension, flushing, restlessness

Dosage: Dilute 1mg in 250mL of D5W; Infuse at 2-10mcg/min or until desired heart rate obtained
Levalbuterol (Xopenex)
Indications: Treatment or prevention of patients over 6yrs old w/ reversible obstructive airway disease

Contraindications: Dysrhythmias with tachycardia

Adverse Rxns: Tachycardia, HTN, hypotension, chest pain, dysrhythmias, tremors

Dosage:0.63-1.25mg nebulized
Pediatric: 0.31mg nebulized
Ibutilide (Corvert)
Indications: Supraventricular dysrhythmias, conversion of afib/aflutter for short time

Contraindications: Hx of heart failure or Vtach

Adverse Rxns: Ventricular dysrthymias, hypotension, HTN

Dosage: 1mg IV over 10 min - may repeat once
Procainamie (Pronetyl)
Indications: Numerous dysrthymias, PSVT, stable wide complex tachycardia of unknown origin, afib with rapid rate in WPW syndrome

Contraindications: 2 or 3 degree block, digitalis toxicity, torsades, complete heart block

Adverse Rxns: Hypotension, bradycardia, AV block, widened QRS, VTach, vfib, asystole, seizure, confusion

Dosage: Pulse - 20mg/min slow IV Arrest - 100mg IV push
Peds - 15mg/kg IV/IO;infuse over 30-60 min
Inamrinone (Inocor)
Indications:Severe CHF refractory to diuretics, vasodilators, and other inotropic agents

Contraindications: Hypotension, valvular obstructive disease

Adverse Rxns: Tachycardia dysrthythmias, hypotension, fever, increased lived enzymes

Dosage: 0.75mg/kg IV over 10-15min - titrate for effect
Metoprolol (Lopressor)
Indications: PSVT, afib/aflutter

Contraindications: Heart failure, 2 or 3 degree block, cardiogenic shock, hypotension, bradycardia, lung disease w/ bronchospasm

Adverse Rxns: Bradycardia, AV conduction delays, hypotension

Dosage: 5mg slow IV at 5 min intervals to a total of 15 mg
Atenolol (Tenormin)
Indications: PSVT, afib/aflutter

Contraindications: Heart failure, cardiogenic shock, bradycardia, hypotension, 2 or 3 degree block

Adverse Rxns: Bradycardia, AV delays, hypotension

Dosage: 5mg IV slow; wait 10min for 2nd dose of 5mg IV slow
Diltiazem (Cardizem)
Indications: Afib/aflutter, multifocal atrial tachycardia, PSVT

Contraindications: 2 or 3 degree block, hypotension, cardiogenic shock, if pt has WPW syndrome, VTach, acute MI

Adverse Rxns: Aflutter, bradycardia, hypotension, chest pain, CHF, syncope, ventricular dysrhythmias

Dosage: 0.25mg/kg IV over 2 min
Labetalol
Indications: HTN emergencies

Contraindications: Bronchial asthma, CHF, 2 & 3 degree block, bradycardia, cardiogenic shock, pulmonary edema

Adverse Rxns: Headache, dizziness, fatigue, edema, ventricular dysrhythmias, allergic, rxns, dyspnea

Dosage: 10mg over IV 1-2min - may repeat or double every 10min; max 150mg
Metaproterenol (Alupent)
Indications: Bronchial asthma, bronchitis, emphysema

Contraindications: Heart dysrhythmias, tachycardia

Adverse Rxns: Tachycardia, dysrhythmias, decreased BP, coughing, tremors, diaphoresis

Dosage: Metered - 2-3 inhalations
Solution: 5-15 inhalations of 5% solution
Dobutamine (Dobutrex)
Indications: Inotropic support for left ventricular dysfunction

Contraindication: Tachycardic dysrhythmias, hypotension w/ signs of shock, drug induced shock

Adverse Rxns: Headache, HTN, ventricular ectopy

Dosage: 2-20mcg/kg/min IV
Ipatropium
Indications: Persistent bronchospasm, COPD exacerbation

Contraindications: Hypersensitivity

Adverse Rxns: Nausea, coughing, headache, tachycardia, dry mouth, blurred vision
