Adenosine

Ind: Symptomatic SVT


Con: Allergy, heart block, drug induced tach, a-fib/a-flutter, wide complex arrhythmia


S.E.: Headache, facial flushing, chest pressure, nausea


Dose/rte: 1st 6mg rapid IVP, 2nd 12mg rapid IVP

Albuterol

Ind: Acute broncospasm

Aspirin

Ind: S/sx of ACS


Con: Allergy, bleeding disorder, already had max of 324 mg


S.E.: Nausea, GI bleeding, rash, tinnitus


Dose/rte: 324mg PO

Atropine

Ind: Symptomatic sinus brady, AV nodal heart block, organophosphate poisoning


Con: Allergy, glaucoma, wide complex heart block, drug induced arrythmia


S.E: Palpitations, tachycardia, blurred vision, dry mouth, ^BP


Dose/rte: 0.5-1mg (cardiac) 2-3mg (organophosphate)

Atrovent

Ind: Bronchospasm, maintenance for pt with COPD


Con: Allergy to drug, soybeans, peanuts; glaucoma


S.E.: HA, dry mouth, tachycardia, N/V, cramping


Dose/rte: 0.5mg in 2.5mL saline nebulized

Benadryl

Ind: Allergic reaction


Con: Allergy, glaucoma, newborn


S.E.: Dowsiness, confusion, N/V, seizure, tachycardia, blurred vision

Dopamine

Glucagon

Con: Allergy, history of tumor that secretes epinephrine

Lidocaine

Ind: Ventricular arrythmias (significant PVCs, VT, VF), anesthesic for pt with pain following IO insertion


Con: Allergy to drug or "caine," heart blocks


S.E.: Seizures, hypotension, bradycardia


Dosr/rte: 1-1.5mg/kg IV/IO, maint 2-4mg/min (ventricular arrythmias), 40mg IO over 2 min

Magnesium Sulfate

Ind: Persistent VF/ pulseless VT, torsades de pointes, eclampsia


Con: Hypotension, heart block


S.E.: Respiratory depression, flushing, drowsiness, muscle weakness, N/V


Dose/rte: 1-2 g over 5-10 min

Morphine

Ind: Suspected cardiac chest pain, CHF, pain management


Con: Allergy, hypotension, head injury w/ ALOC, respiratory depression


S.E.: Decreased LOC, respiratory depression, bradycardia, hypotension, N/V


Dose/rte: 2-10mg IV/IO titrated to effect

Narcan

Ind: Suspected narcotic overdose in pt w/ ALOC


Con: Allergy


S.E.: N/V, tremors, tachycardia


Dose/rte: 0.4-2mg IV/IO/IN (1mL max per nare), repeat 2-5 min, max 4mg

Nutroglycerin

Ind: Chest pain of cardiac origin, angina, acute coronary syndrome


Con: Hypotension (SBP>100 mmHg), erectile dysfunction drugs w/in 24hrs


S.E.: N/V, HA, hypotension, tachycardia


Dose/rte: 0.4 mg sublingual, max 3 doses

Romazicon

Ind: Benzo overdose w/ med control permission


Con: Allergy, poly-drug overdose, chronic benzo use


S.E.: HA, dizziness, dysthymias, seizures, vomiting


Dose/rte: 0.2mg IV/IO, repeat PRN, max 2mg

Sodium bicarbonate

Ind: Known preexisting hyperkalemia, known bicarb responsive acidosis (tricyclic antidepressant OD, ASA OD, cocaine OD, dialysis pt) prolomged resuscitation w/ effective ventilation, ROSC after long arrest


Con: None


S.E.: Alkalosis

Thiamine

Ind: Alcoholics, malnourished patients prior to glucose admin

S.E.: Hypotension, respiratory depression, bradycardia, unrestrained behavior resulting in agitation


Dose: 0.5-2.5mg IV/IO/IN, max 5mg (sedation), 10mg IN (excited delirium), 5-10mg IV/IO, max 20mg (prior to intubation after etomidate), 2-10mg IV/IO/IN, max 20mg

Zofran

Ind: Nausea and vomiting


Con: Known allergy


S.E.: HA, constipation, sensation of flushing or warmth
