What are the hepatic effects of severe hypoxia?
Hepatic autoregulation lost
What are the compensatory mechanism for hypoxia?
Hyperventilation, pulmonary redistribution, increased CO, increased Hb, changes in O2 dissociation curve
What are the compensatory effects of high altitude hypoxia?
Increased # of alveoli, Hgb, myoglobin, decreased ventilatory response to hypoxia
What are the acute responses to high altitude?
Headache, nausea, sleep disturbance, pulmonary & cerebral edema
What is the tx for acute responses to high altitude?
Carbonic anhydrase inhibitor (diamox), steroids and slow ascent
What is hypoxemia?
Deficient oxygenation of the blood (PaO2 < 60 and PvO2 < 30)
What causes hypoxemia?
Decreased PAO2 or true intrapulmonary shunt
Describe the diagnosis of hypoventilation
PaCO2 increased, A-a gradient normal, often responds to 100% FiO2
Describe the diagnosis of absolute shunt (v/q)=0
Normal PaCO2, increased A-a gradient, no response to 100% FiO2
Describe the diagnosis of v/q mismatch
Normal PaCO2, increased A-a gradient, may respond to 100% FiO2
