Antidysrythmic - sodium channel blocker

1.0-1.5 mg/kg, repeated every 3-5 min at half dosage.
Procainamide
Antidysrhythmic - sodium channel blocker

100mg every 5 min at 20mg per min. Discontinue on affect, or 17mg/kg
Adenosine
Antidysrhythmic

6mg rapid IV bolus with flush. 12mg after 1-2 minutes, another 12mg after 1-2 minutes
Diltiazem (Cardizem)
Antidysrythmic - Calcium Channel Blocker

For rapid ventricular rates 0.25 mg/kg IV bolus slow, maintenance of 5-15 mg/hr. For PSVT, 0.25mg/kg slow.
Amiodarone (Cordarone)
Antidysrhythmic - potassium channel blocker

300mg IV, 150mg in 3-5 minutes prn
Magnesium Sulfate
Antidysrhythmic - calcium channel blocker

Vfib/Vtach - 1-2 mg in D5W slow or over 5-30 min for prophylaxis; Torsades des pointes - 5-10mg in 100mL of D5W at 1g/min.
Atropine Sulfate
Parasympatholytic

Bradycardia: 0.5mg every 3-5 minutes up to 3mg; Asystole - 1mg IV; Poisoning: 2-4mg.
Aspirin
Prevents thrombosis

160-325mg PO for chest pain
Sodium Bicarbonate
Alkalinizing agent / buffer

1 mEq/kg body weight IV, followed by half dose every 10 minutes.
Morphine Sulfate
Opioid analgesic

2-10 mg IV (4-6 for pain). 2mg q 2-3 minutes prn for pain.
Nitrous Oxide
Analgesic

Self-administered at 1:4 or 1:2 with oxygen.
Furosemide (Lasix)
Diuretic

20mg slow IV push, 40mg for patients taking lasix - x2 or x3 bolus if needed.
Nitroglycerin
Antianginal

0.4mcg per spray/tablet sublingual, q 3-5 minutes up to x3.
Calcium Chloride
Mineral

2-4mg/kg IV q 10 min prn.
