What are the nursing considerations for a hydatidiform mole?

When a molar pregnancy is suspected, the woman needs emotional support. If a molar pregnancy is diagnosed, the nurse supports the parents as they deal with their grief anout the lost pregnancy.

What follow-up treatment is indicated for a molar pregnancy?
|
Follow up care includes a baseline chest x-ray to detect mets. and a physical exam. Serum B-hCG levels are monitored weekly until the woman has normal titers for 3 consecutive weeks. Titers are monitored for 6 months and the woman avoids pregnancy at this time. If after a year of monitoring, the hCG serum titers remain WNL, a couple may be assured that subsequent normal pregnancy can be anticipated, with a low probablilty of recurring hydatidiform mole.

What is an incompetent cervix?

Refers to the premature dilatation of the cervix.

When is an incompetent cervix most likely to occur?
|
Usually in the fourth or fifth month of pregnancy.

How is an incompetent cervix managed?
|
Traditionally it is managed surgically with a Shirodkar-Barter operation (cerclage) or a modification of it by McDonald - which reinforces the weakened cervix by encircling it at the level of the internal os with the suture material. Once the suture is in place, a cesarean birth may be planned, which allows the suture to be left in place for subsequent pregnancies or it may be released at term and vaginal birth permitted.
