Furosemide (Lasix)

CLASSIFICATION - Loop Diuretic

USES - Treatment of edema associated with CHF, chronic renal failure, or treatment of hypertension.

ACTION - Inhibits reabsorption of Na and Cl, increases excretion of NA, Ca, Mg, Cl, H20, and Ka.

ADVERSE EFFECTS - Polyuria, gycosuria, loss of electrolytes, hyperglycemia, nausea, rash, pruritis (itch), hearing loss

CONTRAINDICATIONS - Allergy to sulfonamides; anuria, hypovolemia, electrolyte depletion.

PRECAUTIONS - Diabetes, dehydration, severe renal disease, cirrhosis, ascites

INTERACTIONS - Cisplatin, vancomycin, lithium
NURSING ASSESSMENT - (1)Assess for hearing loss, tinnitus, ear pain (2)Monitor electrolytes (3)Assess fluid volume (4)Assess B/P

NURSING DIAGNOSIS - (1)Fluid volume, deficient (side effects), (2)Fluid volume, excess (uses) (3)Knowledge, deficient (teaching)

IMPLEMENTATION - Administer 20-80mg q6h po in AM, IM 20-40mg q1-2h (temporary pain at injection site may be noted), IV 20-40mg q1-2h (solution appears clear, colorless)

EVALUATION - Positive therapeutic outcome - (1)Decreased edema (2)Decreased B/P (3)Lowered Ca if malignancy (4)Increased diuresis

PATIENT/FAMILY TEACHING - Expect increased frequency and volume of urination. Report irregular heartbeat, signs of electrolyte imbalances, hearing abnormalities. Eat foods high in potassium such as whole grains, legumes, meat, bananas, apricots, organge juice, potatoes, raisins. Avoid sun/sunlamps.
