Medroxyprogesterone - derivative of progesterone.
I: Contraceptive, DUB
S/E: Acne, weight gain, edema, insomnia, anxiety, ↑ risk for DVT, and cervicitis. ↑ the ratio of LDL to HDL cholesterol.
MOA: Alters gene transcription.
UC: PO. Depo-form is I.M. Contraindicated if prior hx of DVT or history of breast, ovarian or uterine CA
Testosterone - androgenic hormone
I: replacement. Hypogonadism, skeletal growth in pituitary dwarfism.-
S/E: AbN sexual maturation. premature closing of growth plates. Abn lipid levels (↑ LDL, ↓HDL), therefore ↑ risk of atherosclerosis/CAD. Fluid retention, edema,↑ RBC mass may cause ↑ blood viscosity, sludging of blood, & ↑ the risk for ischemia.
MOA: Alters gene transcription
UC: IM (as testosterone cypionate), patch, gel, SL lozenges. Not used in oral form
finasteride - Anti-androgen
I: BPH, male pattern baldness, prostate cancer
S/E: Decreased libido, ED, impotency, depression, breast swelling and tenderness.
MOA: Limits conversion of T to dihydrotestosterone (DHT) by inhibiting type II 5-alpha reductase.
UC: P.O. WOMAN SHOULD NOT HANDLE
Sildenafil citrate - PDE5 inhibitor
I: ED, pulmonary HTN
S/E: "headache, flushing, dyspepsia and nasal congestion. Impaired vision
MOA: inhibitor of cGMP specific phosphodiesterase type 5 (PDE5),
UC: no more than one tablet per day.
Hydrocortisone - glucocorticoid
I: Preferred drug for cortisol replacement therapy
S/E: "↓ resistance to infections, hyperglycemia (mB DM), severe bone loss, (Addisonian crisis)
MOA: Affects gene transcription to either stimulate or repress protein production.
UC: "PO/IV/IM/topical. Chemically identical to naturally occurring cortisol. Short duration of action.
Prednisone
I: Preferred drug for reactive airways disease or mod to severe allergic rxn. Important drug for leukemia reaction.
S/E:

UC: PO. Intermediate duration of action.
Dexamethasone - fluorinated glucocorticoid
I: Extremely potent anti-inflammatory. Also useful in IV form for reducing intracranial pressure.
S/E:
MOA:
UC: PO/IV/topical/inhaled. Long acting duration of action. Minimal mineralocorticoid effects.
DHEA - hormone
I: depression, SLE, and patients undergoing skin grafting. proposed as an “anti-aging” drug
S/E:
MOA:
UC: DHEA use should be ↓ or discontinued when pts experience acne or hirsutism. Contraindicated in pts w/ a hx of sex hormone responsive cancers, such as breast, uterine, ovarian, & prostate cancer.
