What are the charting criteria?
• All written entries are made in indelible ink.
• Highlighting over writing is not used as it
accelerates ink fading and may not be legible
when photocopied; underscoring is permitted.
• All written records are clearly legible.
• There are no blank spaces between entries.
• All pages are in chronological order,
consecutively numbered and dated.
• A consistent format is used throughout the chart
for recording the date e.g. dd/mm/yy, OR
mm/dd/yy.
• All chart entries are recorded as soon as possible
after the patient encounter while the details are
fresh in the Registrant’s mind.
• Generally accepted medical abbreviations may be
used. A legend of abbreviations/codes is in the
appointment record and/or accessible elsewhere
in the office.
