Distal Axon Degeneration (DAD)
exogenous toxins, target axonal transport, dying-back, resembles Wallerian degeneration (myelin sheath breaks down along with axon), muscle atrophy, gradual insidious onset, stocking-glove sensory/motor loss
Segmental (non-uniform) myelinopathy
caused by AIDP, affects internodal mylein and schwann cells themselves, immune attack on PNS myelin, end up with short internodes of myelin
Diffuse (uniform) myelinopathy
CMT!!onion bulb formatiosn from Schwann cells trying to remyelinate
Neuronopathy
cell body of neuron, herpes, polio, drug induced (i.e. pyridoxine-induced), , dorsal root ganglion particularly vulnerable, motor OK, sensory affected, NO REGENERATION
focal (mononeuropathy) vs. multifocal
focal - ischemia, infiltration i.e. tuberculoid leprosy,
classes of nerve injury
Praxia OK, tmesis BAD
1) neurapraxia -sit on leg funny 2) axonotmesis - crush, itnerupt axon, schwann cell basal lamina ok 3) neurotmesis - axon severed |
