What are your indications for a Needle Cricothyrotomy?

There is a need for an advanced airway and ETT or KLT are unsuccessful or contraindicated, or you are unable to ventilate

What are the conditions that need to be met for a needle cricothyrotomy?

  • Pt. is >= 12 years old
  • Pt. LOA is altered
  • no fracture to larynx & or inability to landmark

What is your first step in a needle cricothyrotomy?

You must patch the BHP for permission

What is your first step in initiating a needle cricothyrotomy?

  • Place your patient on his or her back, then extend the head and neck (provided there are no c-spine injuries)
  • neutral positioning may also work as well...

After you have placed the patient on his/her back and extended the head and neck what do you want to do next?

  • Grasp the larynx with your thumb and middle finger.
  • Locate the cricoid cartilage and the cricothyroid membrane with the index finger.
  • Prep the area quickly "iodine or alcohol swab"

After you have grasped the larynx with your thumb and middle finger, located the cricoid cartilage, prepped the area, what do you want to do next?

  • Use the supplied 14g catheter with the 6cc syringe attached.
  • Insert the catheter into the airway at a 45 degree caudal angle looking for free air in the syringe.

After you have inserted the catheter into the airway at a 45 degree caudal angle what do you want to do next?

Remove the syringe and needle leaving the catheter in place?

You have removed the needle and left the catheter in place, what do you want to do next?

Secure the flange of the airway adapter to the patient

You have secured the flange of the airway adapter to the patient, what do you want to do next?

Attach a BVM and attempt to ventilate the patient.
