why do we place an external tocometer when a patient enters labor?

to measure FHR and look at frequency of contractions

what is a normal fetal heart rate?

110 to 160

what is a normal acceleration?

if the HR goes above baseline of 15 bpm for at least 15 to 20 seconds

What does an early deceleration mean?

head compression

nothing to do here really

What about variable decels?

umbilical cord compression- shows no relation to contractions really

Late decels?

fetal hypoxia

this is the most serious and dangerous. shows a clear decrease in HR after the contraction has peaked and then the HR never normalizes until the contraction is complete

What is lightening?

it's the fetal descent into the pelvic brim

What are Braxton hicks?

benign random contractions with no cervical dilation

become more frequent toward the end of pregnancy

what is a bloody show?

sign of cervical effacement

blood tinged mucous from the vagina

so there are these annoying stages of labor to

remember to

What is Stage 1?

onset of labor to full dilation of cervix

What is the latent phase?

it's a subset of stage 1: onset of labor to 4 cm dilation

active phase?

another subset of stage 1: 4 cm dilated--> full dilation

What is Stage 2?

full dilation--> delivery of the neonate

Stage 3?

delivery--> delivery of placenta

What is the difference between primipara and multipara mothers?

multipara mothers have much shorter stages of labor

What is the station?

this where the fetal head is located in relation the pelvic brim

When does the mother push?

during stage 2 when the cervix is fully dilated

all the different steps in delivery may or

may not be important

What is the order?

EDFIEEDD

engagement is

when the fetal head enters the pelvis occiput first

descent is

uterine contractions combined with maternal pushing

flexion

is when the fetus flexes the neck back

internal rotation

fetus's head reaches the ischial spine and they rotate

whatever it's on pg 473

ok

When should we repair lacs?

during stage 3 when awaiting placental delivery

steps for induction

PGE2 is used for cervical ripening

Oxytocin exaggerates uterine contractions

amniotomy- have to look for a prolapsed umbilical cord before doing this
