Contras
Absolute: Head injury; Hypersensitivity; hypotension
Relative: not in out of hospital setting
Precautions
Naloxone should be available to reverse adverse effects
Drug Interactions
CNS depressants, antidepressant, phenothiazines, phenytoin, and MAOI inhibitors poteniate effects
Dont mix w/aminophylline, meperidine, NAHCO3
Preparations
2mg/ml and 10mg/mg ampules and 1mg/10ml preload
Adult dose
Cardiac: 2 to 5 mg IV@ 1 mg/min, rpt. every 3 -5 min. until pain is relieved or respiratory depression occurs
Pain relief: 2-5mg IV loading dose titrated to effect or 5-10mg IM rpt.PRN per protcol
Pedi Dose
0.1 to0.2 mg/kg IV over 5 min may repeat every 2 hours prn
