Sulfamethaxazole, sulfazoxazole, sulfadiazone- MOA, PK, USE
MOA: bact need TH4 to make DNA--sulfa drugs competitively inhibit the precursor thus bacterial DNA cannot be produced
PK: oral unless in combo, enters CSF
Sulfamethaxazole, sulfazoxazole, sulfadiazone- Use, CI, AE
Use: broad spectrum, step pyogenes/pneumo, N. meningitides, nocardia, chlamydia, e coli, UNCOMPLICATED UTIs
CI: PREGNANCY, ALLERGY
AE: kernicterus, crystalluria, stevens johnson, hepatic necrosis, hemolytic anemia, agranulocytosis, aplastic anemia
Sulfamethaxazole, sulfazoxazole, sulfadiazone- MOR
- Increase in PABA production, decrease in drug permeability
TMP and Primethanine- Use, MOA, MOR, AE
Use: wide spectrum, UTIs
MOA: sim to enzyme dihydrofolate reductase-- alone is static
MOR: mutn in enzyme or decreased srug permeability
AE: mimics folate deficiency
PK: oral unless in combo, concentrates in prostate
SMX/TMP- MOA, Use, AE
MOA: synergistic blocking of folic acid synthesis-- is cidal together
Use: wide spectrum, hemophilus, more aerobic GNRs (NOT PSEUDOMONAS)
AE: mimics folic acid deficiency or as w/ sulfa drugs can have kernicterus, crystaluria, severe allergies-- stevens johnson etc
Pyramethamine/Sulfadiiadizine- Use
Toxo
Floroquinolones (floxacins)- MOA, Use, MOR
MOA: bacteriacidal-- inhibits bacterial gyrase, supercoil is maintained.
Use: Prostatitis, UTIs, osteomylitis, DOC PSEUDOMONAS, RTIs, Gram negatives, atypical pneumonia, NO ANAEROBIC coverage
MOR: alteration in DNA gyrase-- gives cross resistance among quinolones, no cross resistance w/ other antimicrobials
Fluoroquinolones- AE, CI
AE: QT PROLONGATION, tendonitis
CI: PEDIATRIC PTS and pregnant pts
Metronidazole- Use, MOA, AE, CI
Use: ANAEROBES, Brain abscesses, aerobes, protozoa (tric, amoeba, giardia), also for C diff superinfection-- GET GAP on the metro-- giardia, entamoeba, trichamonas gardernella, anaerobe, pseudomemb colitis (C diff)
AE: neuropathy, disulfaram rxn
CI: pregnant and CHILDREN
Nitrofurantoin- MOA, Use, AE
MOA: damages DNA via bacterial enzyme that reduces drug
Use: E. coli only, UTI prophylaxis, AE: severe hypersensitivity, acute allergic pneumonitis, severe polyneuropahties
