Bartonella
Orroya fever and Veruana: Invade erythrocytes and endothelial cells causing anemia, swelling, vasoooculsion, and thrombosis. Usually orroya fever (anemia) followed 2-8wks later by eruptive verruga pertuana. Tx: doxy, chloramphenical, blood transfusion |

Trench fever
Cat scratch disease Bacillary angiomatosis-peliosis |
Bartonella
Trench Fever: transmission human body louse with s/s that influenzae like(myalgias, chills, fever, h/a, and bone pain). Tx: doxy or macro (theme crosss cell membrane) Cat Scratch disease: cat or flea bits (1/3 of domestic cats). Infects endothelial cells w/ sx of lympahdenopathy and papule (contact site). Tx; Doxy or macro Bacillary angiomatosis-peliosis: Cats & human body louse that infect immuno-compromised pts. Infects endothelium of capillaries & prodcues bacillary angiomatosis (enlarged cranbery-like papules that may become ulcerated). Peliosis hepatica lesions are cytics blood-filled spaces lined by fibromyxoid matrix (bacteria). Tx: Macro |

Typhus
Rickettsia prowazeckii - rash starting on trunk & migrates to ext. Common in crowded places and wars (thus vaccination of military recruits). Tx: Doxy
Brill's disease
Latent infection reemergence of R. prowazekii
Endemic typhus
Rickettsia typhi - rat flea feces transmission that is prevalent in warm climates. Tx: Doxy or chloramphenical
Scrub typhus
Oreitia - similar to endemic typhus, but no rash. Caused by chigger bite and common in Asia Pacific are.
Spotted fever
Rickettsia rickettsia - RMSF (mid-atlantic states): macular rash begins in extremities and spreads to trunk w/ some petechiae. Transmission through tick bite. Tx: doxy
Ehrlichiae
Target cells are phagocytes and multiply in phagosomes as opposed to cytosol.
Human moncytotrophic Ehrlichiosis (Ehrlichia) deer tick. Likes macrophages and moncytes. Human anaplasmosis: Transmission thourgh same Ixode tick as Lyme disease. Infects neuts and less severe than HME.Look for inclusion bodies in neuts. Tx: doxy |
Q Fever
Influenza-like illness caused by coxiella burneti. Common infection of ruminants and transmission through inhalations of aerosols. Survives in phagolysosome. Low mortality, but serious illness. Tx: doxy or quinolones/
Typhus
Trunk to extermity rash. Tx: doxycycline
Spotted fever
RMSF: Midatlantic and OK/Ark/Mizz
Rash spread from extremities to trunk. |
