chlorpromazine (Thorazine)
antipsychotic used to treat the positive sx of schizophrenia
limited effect on negative sx
what are neuroleptics?
what side effects do they have? why?
1st generation of antipsychotic drugs
called neuroleptics b/c they caused neurologic sx such as extrapyramidal sx
they do this by decreasing dopaminergic activity by blocking the D2 receptor family
chlorpromazine (Thorazine)
thioridazine (Thioril)
mesoridazine (Serentil)
what are they and what are their effects?
low-potency antipsychotics used to tx schizophrenia
more sedative and hypotensive than high-potency drugs
haloperidol (Haldol)
fluphenazine (Prolixin)
what are they and what are their side effects?
high-potency antipsychotics used to tx schizophrenia
extrapyramidal sx are worse than in low-potency drugs
if EPS develop w/ tx of schizophrenia, what can be given to counteract these sx?
anticholinergic agents
switch to a medium-potency or low-potency drug
give an atypical antipsychotic (may be the tx of choice for these pts)
benztropine (Cogentin)
trihexyphenidyl (Artane)
anticholinergics that can be used to tx EPS due to antipsychotic tx of schizophrenia
clozapine (Clozaril)
olanzapine (Zyprexa)
risperidone (Risperdal)
sertindole (Serdolect)
ziprasidone (Geodon)
atypical antipsychotics that can be used to tx schizophrenia if pts get EPS from typicals
the only antipsychotic drug found to be effective in reducing both (+) and (-) sx of schizophrenia
what are 2 other pluses of this drug?
side effect? how might you treat this side effect?
clozapine (Clozaril) - an atypical antipsychotic
produces almost no EPS and reduces depression and suicidality (decreases overall mortality rate)
side effect is granulocytopenia/agranulocytosis
would need to hospitalize to prevent sepsis; may tx w/ filgrastim
when is clozapine (Clozaril) approved for tx of schizophrenia?
what must be done while on this?
only in pts who fail to respond to typicals or who are intolerant to typicals b/c of EPS or tardive dyskinesia
must get WBC count q2 weeks forever
clozapine's strong affinity for ____ and weak affinity for ____, causing its low EPS profile
5-HT2a
D2a
atypical antipsychotic that is more effective than haloperidol in decreasing (+) and (-) symptoms of schizophrenia
risperidone (Risperdal)
newest atypical antipsychotic that has low propensity for EPS, effectiveness in the tx of negative sx, and may have delayed resonse
olanzapine (Zyprexa)
if a patient w/ BDD has concomitant depression, which drugs are first choice? why?
SSRIs b/c they are also effective in treating OCD, which is believed to underlie BDD
first choice of tx for pain disorder
these drugs are often helpful
which drugs should not be given for anxiety?
NSAIDs, NOT opiates
TCAs such as amitryptiline (can be given in low doses for pain)
don't give benzos b/c of potential for habituation (give TCAs)
