orlistat (p. 954)
Alli¸Xenical (weight control agents)

Therapeutic Effect: Weight loss and maintenance in obese patients. Delays onset of type II diabetes

Action: ↓ absorption of dietary fat be reversibly inhibiting enzymes (lipase), which are necessary for the breakdown of subsequent absorption of fat.

Common SE: fecal urgency, flatus with discharge, increased defecation, oily evacuation, oily spotting

RN Considerations: Monitor patient for weight loss and adjust concurrent meds (antihypertensives, antidiabetics, lipid lowering agents)
ranitidine (p. 660)
Zantac (antiulcer agents)

Therapeutic Effect: Healing and prevention of ulcers. ↓ symptoms of gastroesophageal reflux. ↓ secretion of gastric acid

Action: Inhibits the action of histamine at the H2 receptor site located primarily in gastric parietal cells, resulting in inhibition of gastric acid secretion

Common SE: Confusion

RN Considerations: Assess for epigastric or abd pain and frank or occult blood in the stool, emesis, or gastric aspirate, geriatric and debilitated patients routinely for confusion, monitor CBC, may cause false positive results for urine protein
lansoprazole (p. 767)
Prevacid (antiulcer agent)

Therapeutic Effect: Diminished accumulation of acid in the gastric lumen, with lessened acid reflux. Healing of the duodenal ulcers and esophagitis

Action: Binds to an enzyme in the presence of acidic gastric pH, preventing the final transport of hydrogen ions into the gastric lumen

Common SE: dizziness, HA, diarrhea

RN Considerations: Assess for epigatric or abd pain and frank or occult blood in the stool, emesis, or gastric aspirate, may cause abnormal liver function tests, creatinine levels, and electrolytes, monitor PT INR in pts taking with warfarin
misoprostol (p. 869)
Cytotec (antiulcer agent, cytoprotective agents)

Therapeutic Effect: Prevention of gastric ulcerations from NSAIDs. With mifepristone terminates pregnancy of less than 49 days.

Action: Acts as a prostaglandin analogue, decreasing gastric acid secretion (antisecretory effect) and increasing the production of protective mucus (cytoprotective effect), Causes uterine contractions

Common SE: abd pain, diarrhea, miscarriage,

RN Considerations: Assess for epigatric or abd pain and frank or occult blood in the stool, emesis, or gastric aspirate, assess for pregnancy in women who are in child bearing age, monitor uterine cramping and cervix dialtion
metoclopramide (p. 847)
Reglan (antiemetics)

Therapeutic Effect: ↓ N/V, ↓ symptoms of gastric stasis. Easier passage of nasogastic tube into small bowel

Action: Blocks dopamine receptors in chemoreceptor trigger zone of the CNS. Stimulates motility of the upper GI tract and accelerates gastric emptying

Common SE: drowsiness, extrapyramidal reactions, restlessness

RN Considerations: Assess for N/V, abd distension, and bowel sounds before and after, extrapyramidal symptoms (parkinsonian- difficulty speaking/swallowing, loss of balance control, pill rolling, mask-like face, shuffling gate), tardive dyskinesia (uncontrolled rhythmic movement of mouth), neuroleptic malignant syndrome, signs of depression, hepatic function,
