external toco transducer
monitors contractions nd or fetal movement
applied to maternal abd
Baseline FHR
Measured when the uterus is at rest for 10 minutes and rounded to 5 BPM
Variability
•The fluctuation in the baseline FHR.
•Variability occurs because there are many factors which exert a push/pull effect on the fetus and speed up & slow down the FHR.
•FHR is normally irregular –we want to evaluate this
•Described in short term & long term variability
Variability Can be Decreased
Narcotics & sedatives to mom
•Tacycardia
•Prematurity
•Decrease of O2 to fetal CNS
•Abnormalities of CNS
•Fetal Sleep
Accelerations
Abrupt, temporary increase in FHR
•Peaks at least 15 beats per min. above the baseline and lasts at least 15 seconds.
•They often occur with fetal movement.
•They have no r/t to contractions
•Usually a reassuring sign
CVS is recomended for
women with high risk of giving birth to an infant with genetic anomilies
Non stress test
purpose
evaluate the ability of the fetal heart to accelerate
Non stress test
reactive
at least 2 fetal heart accelerations with or wothout movemement, occuring in a 20 minute period and peaking 15 minutes above baseline and lasting 15 seconds
Non stress test
non reactive
tracing does not demonstrae the required characteristicsof a reactive tracing within a 40 minute period
>AFP associated with
-open neura tube defects
-esophogeal obstruction
-abd wall defects
-hydronephrosis
-threatened abortion
-fetal demise
-
> AFP in the normal fetus occurs with
-blood contaminated amniotic fluid
-underestimation of fetal age
-muti fetal gestation
-incorrect maternal weight(lower)
-maternal insuline dependent diabetes
< AFP associated with
-chromosomal trisomies
-gestational trophoblastic disease
< AFP in the normal fetus
-overestimation of gesational age
-incorrect maternal weight(higher)
MSAFP screening
offered to all women 16-18 weeks preg
Ultrasound
Purpose
1 st trimester
- presence and location
-multifetal
-gestational age
- fetal viability
-f/u testing needed
-detects fetal abnormality
-
Ulrasound
purpose
2nd and 3rd trimester
-fetal viability
-fetal anatomy
-progress of fetal growth
-amniotic fluid volume
-location and relation of placenta and umbilical cord
-fetal presentation
-
2nd trimester amnio
done to
detect genetic abnormalities
3rd trimester amnio done to
-fetal lung maturuity
-hemolytic reactions
