Lumbar Strain

-Sudden extension contraction with some type of rotation, chronic strain from posture and mechanics.


-Pain diffuse or localized w/ active extension & passive flexion.


-RICE, followed by stretching & strengthening, NSAIDs, bed rest

Lumbar Sprain

-Forward flexion & rotation, chronic.


-Localized pain lateral to spinous process, pain sharper w/ certain movements or postures.


-RICE, strengthen abs, stretching, trunk stabilization exercises.

Herniated Disk

-Flexion & rotation, in L4-L5 & L5-S1, abnormal stresses & degeneration due to use.


-Episodic, centrally located pain radiates unilaterally in dermatomal pattern, pain when sitting & bending forward, extension reduces pain. Slump test, SLR test, kernig test.


-Ice & stim (pain reducing modalities), manual traction & extension exercises, strengthening exercises, possibly surgery.


-prolapsed: nucleus moves completely through annulus.


-extruded: bulging, contact w/ nerve root.


-sequestrated: nucleus escapes & migrates.

Lumbar Fracture & Dislocation

-Compression Fracture, trunk hyperflexion or falling from a height, dislocations rare, result of direct blow.


-X-ray for detection, point tenderness, palpable defects over spinous or transverse processes.


-X-ray and physicians referral, transport with caution.


SI Joint Dysfunction

-Twisting w/ both feet on ground, stumble forward, fall back, step too far down, heavy landing on one leg, bend forward w/ knees locked during lift; possible anterior or posterior rotation of one side of pelvis; hypomobility w/ pelvic rotation.


-Palpable pain over joint medial to PSIS, pelvic asymmetries, increased pain during side bend, pain radiating posteriorly, laterally or anteriorly down thigh, increased pain w/ unilateral stance. Faber Test, forward flexion test, sacral compression/distraction test.


-Modalities can reduce pain, bracing helpful, SI joint mobilization to correct positioning, strengthening exercises.
