What is the definition of osteoarthritis?
A disease that results in progressive and irreversible breakdown of articular cartilage
Is OA caused by abnormal stress on normal cartilage, or by normal stress on abnormal cartilage?
Both!
What causes cartilage breakdown?
Inflammatory stimulus --> IL-1
IL-1 is proinflammatory, and leads to degradation of cartilage matrix proteoglycans and collagen Cartilage matrix is in a catabolic state. |
What changes occur in the joint with OA?
Articular cartilage: degradation
Subchondral bone: sclerosis, lysis Joint capsule: fibrosis Synovial membrane: hyperplasia, hyperemia, increased synovial fluid production |
Where does pain originate in OA?
Inflamed synovial membrane
Inflamed fibrous joint capsule Inflamed subchondral bone |
What is the difference between SMOADs and DMOADs?
Symptom Modifying OA Drugs - decrease pain
Disease modifying OA Drugs - minimize the progression of OA |
How do NSAIDs help reduce OA pain?
Inhibit cyclooxygenase --> decrease prostaglandins, inflammation
How do corticosteroids help reduce OA pain?
Inhibit phospholipase A2, COX2 --> decreased inflamation
Given intraarticularly |
What can you combine with corticosteroids in joint injections that will help in treatment?
Hyaluronan
Which corticosteroid is anecdotally most effective?
Methylprednisolone
How long should horses be on stall rest after joint injection with corticosteroids?
Typically 7-10 days, followed by a gradual return to exercise over 7 days.
Why is hyaluronon (HA) helpful with OA?
Acts as an anti-inflammatory/mediates joint damage
Decreases chemotaxis, phagocytosis, lymphocyte stimulation, PG release Free radical scavenger May stimulate endogenous HA production. |
Why are polysulfated polysaccharides used for OA treatment? What class of drug are the in?
Considered "slow acting DMOADs"
Prevent, slow, reverse morphologic cartilage lesions Inhibit inflammation Adequan, GAG's |
