What is the normal age of castration?
Typically 6mo-2years
What are the pre-op considerations for castration?
PE: Palpate-- cryptorchid?
Vx: Tetanus toxoid
Peri-op abx: Penicillin
Peri-op analgesia: Bute
What is the open castration technique
Incise the vaginal tunic, retract the testicle/cord
Emasculate cord over incised tunic
What is the closed castration technique?
DON'T incise tunic!
Strip the spermatic cord with gauze, emasculate cord over intact tunic
What is the modified close technique?
Start as closed , strip cord with gauze
Incise vaginal tunic, inspect contents
Emasculate cord over intact tunic
What are the complications of castration?
Bleeding
Eventration/Evisceration
Scrotal/Sheath swelling
Hydrocele
Infection
What are post-op care instructions for castration?
Forced exercise (20 min trot daily)
Paddock turnout (wait 1-2 mo before going out with mares)
Hydrotherapy if necessary
NSAIDs
Abx optional
What is the etiology of a cryptorchid castration?
Hereditary: QH, Percheron, ponies
Improper function of gubernaculum/ insufficient atrophy before descent
What is the Diagnosis of Cryptorchids?
PE
US
Rectal
hCG stim (>100pg/mL stallion, <40pg/mL gelding)
What are the post-op care instructions for cryptorchids?
Perioperative Abx
NSAIDs
Controlled handwalking 2 weeks
Hydrotherapy if necessary
Tetanus immunization
What are the Cast application principles?
Include joint above/below
Extend cast to most proximal aspect of limb segment
IS NOT RIGID FIXATION!
What is the use of a half-limb cast? Pressure points?
Most common- below carpus/tarsus-->foot

Uses: P1/P2 Fractures, Fetlock luxation

Pressure points: Top of cast, sesamoids
What is the use/pressure points of a full limb cast?
Olecranon/just below stifle--> foot

Uses: Cannon bone fractures, luxation of carpus/hock

Pressure points: Accessory carpal bone
Top of cast
Sesamoids
Hock prominences
What are the uses/pressure points of phalangeal cast?
Just below sesamoids to include foot

Uses: Heel bulb/pastern laceration, hoof wall laceration, P3 fracture

Pressure points: Sesamoids, top of cast
What are the uses/pressure points of a tube cast?
From olecranon/below stifle to just above fetlock

Uses: ALD, carpal arthrodesis, carpal/tarsal luxation
Pressure points: Accessory carpal, prox/distal ends
What is the use/pressure points of a transfixation cast?
2-3 4/6 mm positive profile pins at 30* divergence incorporated into cast

Uses: comminuted fractures of lower limb
Breakdown injuries of fetlock

Pressure points: around pins
What are principles of cast monitoring?
STRICT stall rest
Use of limb casted, palpate for heat
Check top of cast for swelling, sores
Check toe for wear
Monitor odor, discharge, staining
What do you do after cast removal?
Depends on original problem
Bandage +/- splint
Stall confinement
Gradual return to exercise
