Steps of TKA

1. BONE CUTS - Cut femur and tibia perpendicular to mechanical axis using either intramedullary or extramedullary guides

2. IMPLANTS - trial implants are snerted to test adequacy of bone cuts

3. BALANCING - Sagittal plane + Coronal plane

4. FINAL IMPLANTATION

What are you balancing in the sagittal plane?

Flexion and extension gaps must be equal

What are you balancing in the coronal plane?

Balancing varus vs valgus deformities by releasing the concave side -->

Varus deformity - release medial side (medial side is too tight)

Valgus deformity - release lateral side (lateral side is too tight)

What is the most common complication?

Patellofemoral complications = patella maltracking, patellofemoral pain, and patellar fracture

What are 2 neurovascular items to watch out for?

Peroneal nerve - esp after mechanical correction of a valgus knee (nerve is stretched)

Superolateral geniculate artery

2 medical complications always to look out for + it's prevention?

DVT and PE; Prophylaxis = prevention = aspirin, warfarin, heparin
