What is a dentigerous cyst?
A cyst that encloses part or the entire crown of an impacted or late-erupting tooth and occurs with comparatively high frequency in humans
In animals there are 3 different lesions that are similar to dentigerous cysts and this has led to confusion
True. There is a cyst similar to that in humans; and ovine odotogenic cyst; and temporal teratoma in horses
Dentigerous cysts commonly occurred in association with the vestigial wolf teeth of horses and vestigial canine teeth of mares
True
A low incidence of dentigerous cysts has been reported in animals with the mandibular 1st premolar tooth most commonly affected
True
What does a dentigerous cyst look like on radiographs?
a well-defined unilocular and radiolucent lesion associated in some way with the crown of the unerupted tooth
Dentigerous cysts comprises ~28% of all jaw cysts in humans and most commonly involves the maxillary permanent canines and mandiublar wisdom teeth
True
Are these cyst lesions more common in females or males?
Males (twice as common in males than in females)
In the literature, molar teeth of the MN/MX and the canine teeth of the MX are frequently reported to be involved and are similar to those in humans
The most common age at onset ranged from 2-5 years
Human literature states the lesion usually originates in the enamel organ of an unerupted tooth, but the exact histogenesis of dentigerous cyst remains unknown
Alternative hypotheses suggest that these cysts arise by proliferation of the outer layers of the reduced enamel epithelium an subsequent cleft formation within the epithelium, somewhat analogous to the change occuring during tooth eruption
Findings in this case report show that the characteristics of dentigerous cysts may be associated with proliferation of basal cells lining the epithelium
True
The epithelium associated with the cyst may arise from one of the following diverse tissues:
1. The epithelium of the tooth crown
2. Epithelial rests of Malassez with cytokeratins showing a wide range of molecular weights
The proliferative epithelium may become enlarged by unicentric expansion due to hydrostatic pressure caused by inflammation
True
The immunohistochemical findings suggested that the biological behavior of dentigerous cyst might be associated with deregulation of cell death in the lining of epithelium of odontogenic origin, along with proliferative activity caused by inflammation
True
Modification to the point of mutation of epithelial cells could occur with chronicity, potentially increasing the risk of odontogenic or SCC arising from dentigerous cyst
A more aggressive and locally invasive character of dentigerous cyst has been described in dogs and classified as Odontogenic tumor
What is the preferred Tx of a dentigerous cyst?
Excision of the entire cyst and systemic antibiotics should be considered
What are the differential diagnoses of a dentigerous cyst?
Dental abscess, odontogenic and non-odontogenic cysts, odotogenic and non-odontogenic tumor/carcinoma, fibrous dysplasia of bone, and other fibro-osseous lesions of the mandible and maxillary bone
???Not understanding the point of immunohistochemistry vs. histopatholgy and the 3 types of cysts??
