What is a fetal non stress test?
it's a general test of fetal well being. It measures the fetal hr in response to fetal movement
The normal response is to see some accelerations
If the fetus is non-reactive, this is non-reassuring
What is the next step if you don't see accelerations on the monitor?
you can do vibroacoustic stimulation
Also, if you get a non-reactive fetal NST then you might start considering emergent C/S or induction
ok
The fetal HR tracing is up on the top
The contraction tracings are the bottom tracing
What would you do for a lady that is not responding well to oxytocin and you get a non-reassuring fetal status?
you might want to proceed with a C/S
Terbutaline can slow down contractions but
would not be good in a patient with bradycardia
What are late decelerations a sign of?
uteroplacental insufficiency
What does a variable deceleration look like on the tracing?
shows an acute fall in FHR with variable recovery
The reason they are variable is that they can show very quick or very long recovery
ok
What causes variable decels?
umb cord compression
What causes early decels?
head compression
Late decels actually begin at or just after the peak
they are due to uteroplacental delivery
This example was a sinusoidal pattern
ok
The true sinusoidal pattern is a regular, smooth, undulating form typical of a sine wave that occurs with a frequency of two to five cycles/minute and an amplitude range of five to 15 beats/minute. It is also characterized by a stable baseline heart rate of 120 to 160 beats/minute and absentbeat-to-beat variability.
ok
What is a good first step if you detect uteroplacental insufficiency on the FHR tracing?
you should put them in left lateral decub position
