What are the steps in the focused history and physical exam/rapid assessment
|
1) Determine OPQRST (history of present illness) and interventions already underway -- ask questions according to the illness
For example: Altered mental status: Description of episode, onset, duration, associated symptoms, evidence of trauma, seizures, fever... Allergic reaction: History of allergies? What were you exposed to? Poisoning: What substance? Time of exposure? How much consumed? Duration of consumption? Weight? Obstetrics: Are you pregnant? How long? Pain or contractions? Bleeding or discharge? Do you feel the need to push? Last period? Crowning? 2) SAMPLE history 3) ASSESSMENT (Responsive=focused assessment, unresponsive=rapid assessment) 4) Baseline vitals 5) CALL MEDICAL CONTROL FOR INSTRUCTIONS 6) Interventions 7) Transport 8) Verbalizes consideration for completing detailed physical examination |

2) Didn't determine scene safety 3) Didn't contact medical direction, follow medical direction, or follow protocols for interventions 4) Did not administer oxygen with proper mask 5) Did not find or manage problems with ABCs or treat for shock 6) Did not differentiate patient's need for transportation vs. continued assessment on-scene 7) Did not ask questions about present illness 8) Administered an inappropriate assessment |
