Doppler ultrasonography: SBP in each arm and in dorsalis pedis and posterior tibial arteries in each ankle (usually similar)

Claudication: 0.41-0.90
Critical ischemia: <0.40
Treatment of peripheral arterial disease?
1. Treat RFs - smoking cessation, statins, ACEi, ASA
2. Pentoxyphylline or cilostazol
3. Assess for revascularization - angio or bypass graft (if debilitating claudication, ischemic rest pain, tissue necrosis)

Also: regular exercise
Buerger disease?
Inflammatory condition of small and medium-size arteries affecting upper or lower extremities

Affects smokers, especially males under 40 yrs
Fibromuscular dysplasia?
Hyperplastic disorder affecting medium and small arteries; generally renal arteries, but also limbs

Usually affects women
Takayasu arteritis?
Inflammatory condition usually affecting branches of aorta, especially subclavian

Causes arm claudication, Raynaud, fever, weight loss

Younger women
SIgns of acute arterial occlusion?
Pain
Pallor
Pulselessness
Paresthesia
Polar
Paralysis
Treatment of acute arterial occlusion?
If threaten limb viability - emergency!

Anticoagulate (heparin), investigate with arteriography
