Why do you give Imipenem with Cilastatin?
Cilastatin inhibits dehydropeptidase (not a beta-lactamase) in the kidney's proximal tubules --> Increase level of impinemem so that it can work against UTIs

Also prevents generation of nephrotoxic metabolite of imipenem
Does Imipenem resist beta-lactamase?
Yes
What are some side effects of Imipenem
Nausea
Vomiting
Increased risk of C. difficile infection (compared to other Carbapenems)
Seizures (1.5%)
What are Doripenem used for?
Complicated intra-abdominal infections
UTIs
What bacteria is extremely resistant to Carbapenem? Where are they usually found?
Carbapenem-resistant Klebsiella pneumonia (CRKP)

Emerges in long-term care facilities, nursing homes, elderly
What is the Rx interaction for Carbapenems?
Decreased levels of valproic acid --> Increased risk for seizures
