Main fxns of IABP
increase perfusion to coronary arteries
decrease afterload
Secondary fxns of IABP
decrease workload of heart
decrease myocardial O2 demand
counterpulsation
volume displacement created by rapidly shooting helium in and out of balloon
normal adult IABP balloon size (volume)
40 ml
Normal IABP placement location
tip 1-2 cm below L SC artery and above renal arteries
(CXR-btwn 2nd & 3rd intercostal space)
what is the augmented diastolic wave form & what do you compare it to?
It is the wave created when balloon inflates and it should be higher than the patient's unassisted systolic
what is the augmented end diastolic wave and what do you compare it to?
This is the wave created after the balloon has deflated at end diastole. IT should be lower than the patient's own diastolic pressure d/t sxn created when balloon rapidly deflates
what is assisted systole and what do you compare it with?
It is the patient's systolic peak wave after balloon pump has inflated and deflated and afterload has been reduced so it should be lower than the unassisted peak systolic
When should IABP inflate?
onset of diastole (diacrotic notch or peak of T wave)
diacrotic notch
notch in arterial waveform when semilunar valves shut (aortic and pulmonic valves). Marks start of diastole
Name the waves
A= unassisted end diastolic
B= unassisted peak systolic
C= diacrotic notch
D= assisted peak systolic
E= augmented diastolic
What IVF should be pressurizing the central lumen of the IABP?
1000 units heparin in 500 ml bag of 1/2 NS
What sign tells you IABP may have been inserted too high? too low?
too high= absent left radial pulse
too low = abnormal u/o (bloody, decreased)
Balloon inflation pressure should not be more than _____
40 ml
hallmark sign of IABP rupture is:
blood in the connecting tubing, this requires immediate cessation of IABP and lowering of HOB and call MD,
DO NOT let blood fluid reach IABP console!
IABP pressure readings must be recorded every ____ in EPIC
hour
