What is the one thing you need to assess every time you see a HD patient?
Risk for suicide
What is one of the central cognitive deficits seen in HD patients
Along with dementia, you see a decrease in processing speed
What is the presentation of Juvenile HD
Myoclonus, Seizures, Parkinsonism, Cognitive change. Choreiform movements are typically absent.
What can be used for diagnosis of HD?
Family history
Physical exam
Genetic testing
Neuroimaging (caudate atrophy)
What is the treatment program for HD?
Currently, there are no disease modifying agents for HD
What is the breakdown of treatments for HD?
10% medicine and 90% supportive and behavioral
What is the first primary step/referral for an HD patient?
With the Huntington Disease Society of America
Are there any treatments for cognitive symptoms?
What drug should theoretically work?
No, there are no current treatments for dementia
Namenda should theoretically work since it's an NMDA receptor agonist, but there aren't many studies on it
Where is the focus for cognitive symptoms treatment?
On non-pharmacological treatment
What is used to treat psychosis or agitation in HD patients?
Typical antipsychotics, such as Quetiapine.

(Risperiodone, haloperidol = worse cases)
What is the treatment for chorea?
Non-pharmacologic treatment mainly, first determine if the patient requires treatment, chorea movements often don't bother the person. Mainly, provide a calm stable environment
What is the one pharmacologic treatment for chorea?
Tetrabenazine which is a central monoamine depleting agent: it inhibits presynaptic dopamine release and blocks CNS dopamine receptors. It is useful in milder forms of chorea. It may worsen cognitive problems.
What is another pharmacologic treatment for chorea that treats both psychiatric and motor symptoms?
Neuroleptics which can block dopamine transmission at the post-synpatic receptor site and can be used to treat both the psychiatric and motor symptoms in HD, although they can worsen the parkinsonian features.
What atypical antipsychotics can be used to treat chorea?
Zyprexa, Abilify and Risperdal are better for mild to moderate chorea.
What typical antipsychotic can be used to treat chorea?
Haloperidol is better for moderate to severe chorea
What NDMA receptor antagonist drug could be used to treat chorea?
Amantadine
What is one investigational pharmacologic treatment for HD?
Creatine, to reduce oxidative stress and slow apoptosis
What is one surgical investigational treatment for HD?
DBS: deep brain stimulation of Globus Pallidus interns
