Why would maternal dehydration cause fetal tachycardia?
Maternal CO drops with dehydration so her heart rate will increase to compensate, so will the baby's
Why would you be very cautious giving pain meds to a pt with EFM showing low variability?
Pain meds will decrease variability and this fetus is already low
What are you thinking if EFM has absent variability?
CRAP!!! this is non-reassuring and you cannot rule out fetal acidosis
Incidence of seizures
Risk of having a seizure at some point in your life is 10%
70% of new cases have no identified cause
Incidence of epilepsy
highest at < 2yo or > age 65, males > females, minorities > caucasians
Sodium and seizures
Any alteration in sodium channel can result in a seizure, but more likely when sodium is very low
Calcium and seizures
Extracellular calcium is what is needed to activate a seizure
Inclux of Ca through T channel leads to the generation of electrical current, certain neurons in the hypothalmus have T currents large enough to cause action potential firing
GABA and seizures
GABA is a widely distributed inhibitory NT with the job of calming activity around the electrical impulse so it doesn't spread.
What are the most frequent known causes of seizures?
Head trauma
Brain tumor and stroke
Poisoning
Infections
Maternal injury
Some women have premenstrual seizures
Is a client breathing during the tonic phase of a seizure?
NO! 10-30 seconds, cant breath b/c diaphragm muscle has contracted
Why is status epilepticus a medical emergency?
b/c the brain is not getting all the O2 it needs --> brain injury or death, person also becomes acidotic. Risk increases with longer seizure
What will you give to treat status epilepticus?
Benzo's (ativan or valium) by slow IV push to activate GABA receptors and calm the brain
Bolus IV anticonvulsant and then intermittent doses
***Antiseizure meds work in what 3 ways?***
1. Suppress sodium
2. Inhibit calcium
3. Potentiate GABA
What might happen if dopamine is antagonized?
may initiate a seizure
