-Stick tongue out and check for symmetry. Have pt put tongue against cheeks on both sides and resist. -Pharanx, palate lifting- "Ah." Inspect posterior pharanx. |

-On PE the pt is sitting comfortably in NAD.
-He appears his stated age. -BP is 120/80. Pulse is 60. RR is 14. -Head is NC & AT. -Upon eye exam, conjunctiva were clear, sclera non-icteric, no ptosis, no lid lag or lid erythema. -Vision is 20/20 on left and 20/20 on right with glasses. -EOc movements are intact -P are ERRLA -Visual fields are within normal limits bilaterally. -Fundi are w/o hemorrhages or exudates with sharp disc margins bilaterally w/o papilledema. |

-Upon ear exam, gross hearing intact bilaterally.
-Normal Rinne and Weber tests. -External canals clear w/ normal architecture and w/o erythema bilaterally. -TM translucent, no erythema, dullness, or bulging, and with normal cone of light bilaterally. -Upon nasal exam, septum is midline, pink mucosa w/o tenderness or swelling -No sinus pain or tenderness. |

-Upon oral exam, there were moist mucus membranes, no oral lesions, good dentition.
-No tonsillar hypertrophy, erythema, or exudates. -Tongue/uvula midline -Symmetric elevation of soft palate. -Upon neck exam, neck is supple and freely movable w/o lymphadenopathy, tenderness, thyromegaly, or mass. -Trachea is midline. |
