HPV risk factors
# sexual partners, age of first intercourse, OC use, age at first preg, hx of sti, HIV, steroid use, immunosuppressive meds, cigarettes, poor ed, low socioeconomic status, non-white, screening noncompliance
HPV diagnosis
screening w/ PAP smears
late signs: postcoital or intramenstrual bleeding, discharge, , hematuria or incontinence, vaginal hemorrhage, flank pain (ureter obstruction), leg pain (sciatic nerve involvement), cachexia
Endometrial Cancer subtypes
I - hormonal imbalance (obesity, nulliparous, HTN +/- DM, early menses, late menopause, no OC use, tamoxifen, thyroid dz)
II - not hormonal, similar to ovarian cancer, no precursor lesion
Endometrial Cancer Dx & Tx
no screening test
peri or post menopausal bleeding (early stage, good prognosis when caught early)
ultrasound to measure endometrial thickness, followed by biopsy if necessary, D&C
Tx: hysterectomy + bilat salpingo-oophorectomy + lymph node dissection +/- radiation
Ovarian Cancer types
Epithelial (90%) - serous, mucinous, endometrioid ,clear cell, undifferentiated, transitional cell (brenner)
Borderline (10-15%) - good prognosis
Germ Cell (2-3%)
Stromal (7%)
Primary Peritoneal (acts like epithelial)
Ovarian Cancer risk factors
early menarche, late menopause, nulliparity, infertility, ovulation induction meds, family hx
protective: OC, pregnancy, breast feeding, tubal ligation, hysterectomy, oophorectomy
Ovarian Cancer Dx & Tx
no screening test, no signs & symptoms --> caught in late stages, pay attention if see these in women in 60s,70s,80s!!!
dx: abdominopelvic mass, ascites, vaginal bleed, bloating, distension, GI, urinary, or resp symptoms, definitize w/ laparotomy or laparoscopy 70% stage III 15% stage IV
tx: hysterectomy + bilat salpingo-oophorectomy + lymph node dissection + omentecomy +/- chemo
5 yr survival = 40% b/c of late dx
Ovarian Cancer Genetics
BRCA 1 vastly increases risk
tx pt w/ OC until children desired, preemptive hysterectomy + bilat salpino-oophorectomy after child bearing
